Job Title: Health Home Care Manager Full-Time (New York State)
Location:Brooklyn NY (Field-based & Hybrid within New York State)

As a lead Health Home provider we are looking for dedicated professionals to join our growing Care Management team.
Position Overview
We are seeking aHealth Home Care Managerto provide comprehensive person-centered care coordination for Medicaid-enrolled individuals with chronic medical and behavioral health conditions. This position involves field visits ongoing communication with patients and providers and collaboration across clinical and social support systems to ensure access to essential services and supports.
Key Responsibilities
- Conduct comprehensive assessments and develop person-centered care plans
- Coordinate services across medical behavioral health substance use and social service providers
- Monitor progress toward goals through in-person and remote contacts
- Support patients with appointments medication adherence housing benefits and self-management
- Document all contacts and activities in electronic health record (EHR) systems in a timely accurate manner
- Maintain compliance with Health Home standards andNew York State Department of Health (DOH)guidelines
- Collaborate with interdisciplinary care teams including family and community partners
Qualifications (Per NYS DOH Guidelines):
You must meetone of the following:
- Bachelors degree 2 years of care coordination or related experience
- Licensed RN 2 years of experience
- Masters degree in health/human services 1 year of experience
Plus:
- Strong knowledge of Medicaid populations behavioral health and community-based care
- Excellent interpersonal time management and documentation skills
- Proficiency in Microsoft Office and/or EHR systems
- Ability to travel locally within New York State for home/community visits
- Bilingual (Spanish Haitian Creole Bengali etc.) a strong plus
