Accountabilities:
- Work collaboratively with the Pharmacy Team to provide patient-centred care
- Dispensary duties including medication order entry and medication profile review
- Provide drug information to physicians nurses and other clinicians to enhance patient care
- Verification of all medication orders for inpatients
- Provide therapeutic drug monitoring
- Participate on hospital committees as required
- Perform and/or supervise in patient dispensing (which includes computerized order entry unit dose centralized IV admixture services TPN Chemotherapy and an understanding of automated systems) drug information and administrative activities as assigned
- Resolve day-to-day clinical issues
- Assist with the implementation of pharmacy initiatives which improve patient care and other initiatives which improve the efficiency and effectiveness of the Pharmacy operations
- Other duties as assigned
Qualifications :
- Currently licensed (or eligible) by the Ontario College of Pharmacists Part A of the registry
- Relevant clinical experience preferable
- Excellent attendance punctuality and work record
- May be required to travel between and work at all Osler sites
- Excellent computer skills
- Excellent oral interpersonal and written skills
- Demonstrate initiative and a willingness to work collaboratively in a team environment
- Strong clinical assessment skills in identifying and resolving drug-related problems
- Demonstrated ability to apply clinical judgment in analyzing and solving clinical problems
- Meditech computer experience preferable
- Must be able to meet the physical demands of the position

Additional Information :
Hours: Evenings Monday to Friday (subject to change in accordance with operational requirements)
SALARY:
Minimum: $50.51 per hour
Maximum: $64.54 per hour
