Job Description:

Statement of Purpose:
To provide administrative support to the Residential/Day Program
Manager/Supervisor and to provide support and advocacy for
residents/participants in meeting their goals and needs


Accountable to: Residential/Day Program Manager/Supervisor or designate


Qualifications: 1) Completed Developmental Services Worker diploma or other degree/diploma
related to human services
2) Three (3) years full-time experience working with people who have
developmental disabilities
3) Experience with case management or prime worker role and responsibilities
4) Demonstrated ability and experience in Individual Support Planning
5) Experience in program (teaching/training plans or behavior management
programs and reinforcement schedules
6) Teaching skills (demonstrated ability to implement various teaching approaches
and/or group instructor skills)
7) Proficient in medication administration
8) Demonstrated leadership/administrative aptitude/abilities
9) Valid Standard First Aid certificate (including CPR)
10) Nonviolent crisis intervention and behaviour management skills.
11) Ability to work independently and as part of a team to provide a supportive and
nurturing environment
12) Computer skills (Microsoft Office internet)
13) Training and/or experience in personal care (i.e. assisting with G-tube feedings
continence devices) is an asset and willingness to be trained
14) Training and/or experience in supporting individuals through ambulates
mobilizers transfers is an asset
15) Experience in community outreach is an asset
16) Valid drivers license is an asset

RESPONSIBILITIES

PROGRAM

1) Know and respond to safety and health care needs of residents/participants
2) Act as a role model for all residents/participants as reflected by appearance behaviour and
attitude
3) Perform Prime Worker/Caseload duties as assigned by Residential/Day Program
Manager/Supervisor
4) Provide support for residents/participants personal development as decided through Individual
Support Planning
5) Supervise and assist residents/participants with meal preparation and clean-up
6) Administer medication as prescribed and according to policies and procedures
7) Support resident/participant involvement in activities (Faith Finders bowling special events etc.)
8) Complete Shift Reports/Daily Logs and review/update Communication Book
9) Maintain pertinent resident/participant records
10) Support family involvement with residents/participants site activities and special events
11) Follow and promote the agencys and site program goals and philosophy
12) Liaise with transportation companies day program staff group home staff other professionals
families etc.
13) Ensure learning opportunities are provided using agency and community resources

STAFF

1) Maintain effective communication
2) Assist in orientation as directed by Residential/Day Program Manager/Supervisor. Provide support
and guidance to coworkers placement students volunteers and/or external agency staff
3) Represent the site on committees and at other meetings as delegated by the Residential/Day
Program Manager/Supervisor
4) Attend and participate in meetings and staff development training at the discretion of the
Residential/Day Program Manager/Supervisor and Administration
5) Counsellor-In-Charge (CIC) in the absence of Residential/Day Program Manager/Supervisor (seeCIC description below)

HOUSEHOLD ORGANIZATION

1) Adhere to site menu plan and food restrictions
2) Ensure the general site appearance is clean and tidy before the shift is completed
3) Maintain and account for use of site and resident monies TTC tokens taxi chits household supplies
etc.
4) Complete daily household routines as required (laundry cooking cleaning etc.)

HEALTH & SAFETY

1) Comply with the provisions of the Occupational Health & Safety Act and regulations
2) Adhere to the agencys Health & Safety policies and program
3) Complete daily walkthroughs/inspections of site as assigned
4) Report complete and forward maintenance requisition forms to main office
5) Report accidents/injuries/workplace hazards in a timely manner
6) Responsible use of the Agencys vehicles equipment and property
7) Follow the principles of nonviolent crisis intervention philosophy

ADMINISTRATIVE
1) Assist in completion of staff pay sheets/Ulti-Pro as assigned by Residential/Day Program
Manager/Supervisor
2) Assist in recording vacation and sick accruals/use as assigned by Residential/Day Program
Manager/Supervisor
3) Scheduling in consultation with Residential/Day Program Manager/Supervisor
4) Update Rotational Relief list
5) Ensure household/program inventory is maintained in consultation with the Residential/Day
Program Manager/Supervisor
6) Ensure supply of TTC tokens taxi chits office and medical supplies etc.

ORGANIZATIONAL

1) Adhere to standards that are consistent with the values and philosophy of The Salvation Army and
Broadview Village
2) Be familiar with and adhere to Salvation Army policies and procedures
3) Participate fully and constructively in meetings and in-service training as required
4) Participate in regular PEAC coaching/supervision and consultation with supervisor
5) Participate in formal PEAC annual review sessions with supervisor
6) As required and is applicable make suggestions that will improve efficiency working conditions or
procedures to management
7) Represent the organization in a professional manner

Statement of Purpose:
The role of the C.I.C. is to oversee the program needs in the absence of the
Residential/Day Program Manager/Supervisor of the Residential/Day Program Manager/Supervisor as described below:

SHIFT

1) Direct delegate coordinate organize and ensure the completion of shift responsibilities
2) Ensure relevant information is communicated
3) Ensure adequate staff coverage to meet the needs of the program. In the event of staffing shortage
C.I.C. is responsible for the coverage of the succeeding shift until back up staff can arrive.
4) Ensure daily logs and program data are completed.
5) Ensure incident reports are completed and forwarded to main office
6) Ensure emergency procedures are followed
7) Ensure that medications Policies and Procedures are followed

HEALTH & SAFETY

1) Comply with the provisions of the Occupational Health & Safety Act and regulations
2) Adhere to the agencys Health & Safety policies and procedures
3) Ensure the completion of daily walkthroughs/inspections of site

The position of Team Leader encompasses Prime Worker/Caseload duties for assigned
residents/participants:
A Attendance and participation in all program meetings and/or agency events as applicable
i. Team Meetings
ii. Admission/Transfer/Discharge meetings
iii. Annual Open House
iv. Individual Support Plan Meetings
v. Program Review Meetings (monthly) / PA Days
vi. Consultations with other professionals
vii. Other events and/or meetings as assigned
B Completion and presentation of information for program related meetings

PHYSICAL EFFORT/DEXTERITY REQUIRED: The performance of the job requires bending/twisting/lifting
of 50 lbs considerable walking; standing; writing; computer work; and light housekeeping.


WORK ENVIRONMENT: The incumbents work environment is typically in a group home or day
program/classroom type facility.


The incumbent works under generally agreeable conditions with the following exceptions:
Exposure to dirt bodily fluids bodily waste Noise
Dealing with angry and abusive people Verbal abuse
Handling garbage Odours


EXPOSURE TO HAZARDS: The incumbent may be exposed to risk of minor to moderate injury or illnesses
due to the following:
Physical attacks by others (i.e. being attacked by
a participant)
Exposure to infectious diseases
Exposure to toxic chemicals Exposure to bodily fluids
Sharp objects Other dangerous situations


PRECAUTIONARY MEASURES: The incumbent must take the following precautionary measures (in
addition to Health and Safety rules):
Universal Precautions WHMIS Training
First Aid certification Police/Criminal Reference Check
Vulnerable Sector Screening
Nonviolent crisis intervention training
i. Assessments
ii. Program Reviews and Data Summaries including graphs
iii. Individual Program Plan conference package
iv. Reports applications and consents as required
v. Other documentation as required
C Development implementation and evaluation of the Individual Support Plan (ISP) / individualized
programming in consultation with the resident participant Residential/Day Program
Manager/Supervisor the site program team and other members of the ISP Planning Team
D Ensure the following for residents/participants as applicable
i. Health Care appointments in accordance with Ministry and agency standards
ii. Leisure and vacation activities in accordance with resident/participant preferences and needs
iii. The purchase suitability and maintenance of residents clothing
iv. A current inventory of residents belongings
v. Responsible use and documentation of residents finances
vi. Support and/or coordination of day activities
vii. Relationships with family residential programs day programs peers friends volunteers and
others
viii. Support and coordination of outreach and vocational opportunities
ix. Representation/advocacy for monthly planning (i.e. BVEP calendar of events)

Compensation:

The target hiring range for this position is $28.27 to $29.31 with a maximum of $30.35.

Placement in the salary range will be based on factors such as market conditions internal equity and candidate experience skills and qualifications relevant to the role.
