Job Description
Per Diem Pharmacist Greenlawn Long Island New York - OFFS
Job Title: Per Diem Pharmacist
The Per Diem Pharmacist serves patients by preparing medications; giving pharmacological information to multidisciplinary health care team; monitoring patient drug therapies.
Roles & Responsibilities:
- Dispenses medications accurately and in a timely fashion.
- Resolves questions regarding formulary and non-formulary medications following policies and procedures.
- Provides dosing and therapeutic recommendations that take patient age/kinetic based on literature.
- Performs clinical chart reviews on a scheduled basis.
- Performs Patient Care Unit inspections on a scheduled and timely basis.
- Prepares parenteral medications using sterile aseptic technique.
- Maintains adequate medication supply in the Pharmacy area and informs supervisor of impending shortages
- Intervenes and documents interventions to correct/ improve pharmacotherapy.
- Reports adverse drug events and medication errors occurring in area serviced.
- Takes responsibility for the operation of the assigned Pharmacy area and activities of all supportive personnel.
- Maintains pharmacological knowledge by reading professional publications attending workshops conferences and C.E. programs to maintain professional competency.
Requirements:
- Graduate from an accredited College of Pharmacy (ACPE).
- 1 year minimum as licensed Pharmacist
- Licensed Pharmacist-preferred
