DescriptionDemonstrates the knowledge and skills necessary to accurately evaluate and effectively treat all assigned patients. Devises a treatment plan that addresses individual age specific needs incorporating patient goals to emphasize functional restoration and quality of life. Performs initial examinations and provides physical therapy intervention to a diverse patient population. Works under the supervision of the Therapy Manager/Senior Therapist. (Inpatient Rehab and Acute) This position is entry-level and part of a multi-disciplinary team and has the opportunity to rotate across Inpatient Rehab and Acute services.
Responsibilities- Monitors appropriateness of goals and prioritizes treatments to maximize patient benefit.
- Educates the patient and their family members regarding home exercise program diagnosis specific information and purpose of any therapeutic intervention
- Coordinates with other services when scheduling patients and adjust schedule as necessary to provide additional coverage
- Accurately completes all required documentation according to guidelines
- Reports patient information at chart rounds conferences and family meetings
- Maintains the clinic in proper working order and assists in monitoring of inventory
- Participates in the supervision of Assistants students Attendants and volunteers
- Attends all department meetings as required
- Performs initial examinations establishes appropriate goals plans and implements treatment interventions performs timely re-examinations and modifies goals and treatment interventions accordingly. Coordinates with other disciplines when indicated.
- Assumes responsibility for all documentation requirements in a timely manner i.e. initial examinations progress notes monthly summaries and discharge summaries.
- Provides training to families or care partners and medical personnel as indicated.
- Recommends appropriate durable medical equipment in coordination with appropriate team members. Writes letters of medical necessity when indicated.
- Participates in discharge planning makes recommendations for continued physical therapy and provides home exercise programs when appropriate.
- Attends all appropriate conferences rounds in-services and meetings.
- Will be assigned additional patients in the event that coverage is necessary as dictated by staffing needs.
- Maintains therapy attendance records patient charges and statistics.
- Participates in the supervision of Rehab Patient Care Associates volunteers and students.

QualificationsEducation
- Masters degree. Doctorate preferred
- Name: Current New York State Physical Therapy license. Graduate of an accredited PT Curriculum Issuing Agency: DOH/Office of Professions
