About the Role
You will work within a flexible multidisciplinary structure that ensures the delivery of high-quality assessment and management services to Memory and Geriatric clients making sure that the clients and families needs are met in a timely manner. The role is currently based at St Georges Hospital Kew. You will conduct triage comprehensive pre-clinic assessments develop the clients care plan and provide ongoing education and service linkage for clients and families as appropriate.

You will be the patient and familys contact person throughout their journey


Classifications for this position will range from classification range VG3 - VG6 ($104135.20 - $118981.20 per annum).

Your Contribution

  • You will have a Bachelor of Allied Health discipline (eg Social Work Occupational Therapist Speech Pathologist) or equivalent or Bachelor of Nursing with minimum of 7 years of Allied Health Aged Care or Nursing experience with dementia and/or psychiatric illness in aged clinical populations in a community based multidisciplinary setting.

  • Extensive knowledge and experience of dementia syndromes and psychiatric illness in aged clinical population and their management.

  • Detailed knowledge of dementia and aged care service systems and community supports for clients/carers.

  • Demonstrated ability to work in multidisciplinary teams at an advanced level.

  • High level of understanding of the impact of a diagnosis of dementia on the individual and their families

  • Highly developed assessment skills Ability to provide post diagnosis education counselling and future care planning to families/ carers.

  • Highly developed individual family work and crisis intervention skills

  • Highly developed interpersonal skills. These should incorporate qualities of excellent communication (verbal and written) and skills in collaboration negotiation and conflict management

  • Current National criminal history check or willing to obtain

About CDAMS and Geriatric Medicine Clinics - Health Independence Program (HIP)
The CDAMS & Geriatric Medicine Clinics are part of the HIP Specialist Clinics. Services may be provided in the clients home or in the centre. The role involves interaction with GPs medical specialists clients family and carers other SVHM and HIP services.

WORK ENVIRONMENT

This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for cliennts and to support hospital demand management and flow through delivering integrated and coordinated care to clients across the hospital and community interface reducing duplication and fragmentation of services and ensuring client centred care planning.

SVHM HIP consists of integrated hub sites across several locations including in Fitzroy Kew North Richmond and Fairfield.

This position will be part of HIP Access Rehab and Clinics stream based in the Cognitive Dementia & Memory Service (CDAMS GMC) Team. This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.

HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH)

PURPOSE

To work within a flexible multidisciplinary structure that ensures the delivery of high quality assessment and management services to Memory and Geriatrcic clients making sure that the clients and families needs are met in a timely manner. The Memory & Geriatric Medicine Key Worker is responsible for being the key contact person for clients and families whilst attending the clinic. The Key Worker conducts the pre-clinic assessment for their clients develops the clients care plan and provides ongoing education and service linkage for clients and families as appropriate. The role covers the Memory & Geriatric Medicine Team Leader role during absences including the day to day coordination of the program and problem solving issues

DUTIES

  • Act as key contact for memory clients and geriatric medical clinic clients
  • Provide efficient high quality triage and timely assessments treatment plans and management of clients throughout the continuum of care in the most appropriate location based on the clients needs.
  • Provide high quality education counselling and future planning to clients carers and families
  • Use a wide range of clinical skills to enhance the client and familys experience of this clinic and achieve positive client and family outcomes
  • Assess manage and discharge all assigned clients according to procedures and relevant protocols.
  • Communicate client issues effectively with other team members and lead / chair multidisciplinary team meetings.
  • Timely liaison of issues with team members and identify and implement solutions
  • Supervision guidance and support to Grade 2 Key Workers
  • Provide cover for Memory & Geriatric Medicine Team Leader in terms of ensuring smooth day to day operation of service efficient use of resources allocation of clients and problem solving issues.
  • Establish develop and maintain strong and positive working relationships within the HIP team referral sources participating agencies and other service providers.
  • Ensure optimal use of clinical capacity regularly monitor capacity including for any gaps that may occur due to late changes/ cancellations to client appointments.
  • Actively lead quality improvement initiatives (includes working groups projects audits continuous improvement initiatives) to improve the service and Health Independence Program
  • Achieve set activity targets and complete activity statistics by end of working week or end of month (whichever comes first)

CRITERIA

  • Bachelor of Allied Health discipline or equivalent or Bachelor of Nursing or equivalent
  • Post graduate qualifications highly regarded
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) (if required)
  • Eligibility for membership of relevant discipline association
  • Evidence of ongoing education/professional development relevant to cognitive dementia and memory services and/or Aged Care.
