Overview
Star Nursing is expanding its Advanced Wound Care Program and hiring Nurse Practitioners (NPs) to lead in-home and facility-based wound consultations. Training is provided. This mobile role focuses on delivering high-quality patient-centered wound care in private homes assisted living and independent living facilities.
Your work will contribute directly to better outcomes for Californias aging and medically complex populations with future potential for expanded primary care responsibilities.
Key Responsibilities
- Conduct in-person wound assessments including history physical exam and staging
- Perform wound care procedures: debridement dressing changes topical applications
- Create and manage individualized wound care plans
- Coordinate with interdisciplinary teams and support discharge planning
- Educate patients and caregivers on wound care and prevention
- Maintain complete timely and compliant medical documentation
- Use and understand Durable Medical Equipment (DME) relevant to wound management
- Stay current with wound care innovations and training opportunities
Required Qualifications
- Current California Nurse Practitioner (NP) license and RN license
- Furnishing number for prescriptive authority under CA law
- DEA registration for controlled substances (if applicable)
- Valid California drivers license auto insurance and reliable transportation
- BLS certification required; ACLS preferred
Preferred Experience
- 2 years of experience in wound care
- 2 years working with senior or geriatric populations
- 2 years in outpatient or home-based care
- Experience with EHR systems and clinical documentation
- Bilingual in Spanish or Tagalog is a plus
Physical & Travel Requirements
- Frequent local travel across assigned counties
- Ability to carry up to 25 lbs of medical supplies
- Capable of standing bending kneeling and providing care in non-clinical settings
