- Assesses injuries and implements or recommends treatment as allowed by license or delegation
- Acts as liaison with participant families physicians pharmacies and other involved agencies
- Works closely with the referring entity Medical Director and Manager of Medical Services to ensure proper case coordination
- Assists the treatment team in monitoring the implementation of the treatment plan to assure services are being provided according to plan
- Schedules plans and gathers information for the treatment plan meetings
- Assists Direct Clinical Management with report writing and distribution in a timely manner
- Documents all clinical and case management activities conducted on behalf of the participant according to program company participant legal and regulatory requirements and timelines
- Manages collection and storage of participant and program records according to program company client legal and regulatory requirements and timelines
- Participates in on-call crisis rotation
- Prepares and implements family visitation if applicable
- Works cooperatively using a team approach
- Involves the guardians and/or caregivers in as many decisions about day-to-day care as possible
- Provides services that respect each participant and familys cultural background heritage and identity
- Proactive and engages in public relations with individuals families employees and community members demonstrating professionalism at all times
- Coordinates all needed documents for audits
- Conducts program tours for visitors families and other involved agencies
- Represents the company at conferences job fairs and community events if needed
- Maintains the highest standards of professional conduct in relation to information that is confidential in nature. Shares information only when recipients right to access is clearly established and sharing of such information is in the best interest of the participant
- Adheres to and participates in Companys mandatory Health Insurance Portability Accountability Act privacy program/practices and Business Ethics and Compliance programs/practices
- Abides by and demonstrates the companys Mission Vision and Values through both behavior and job performance on a day-to-day basis
- Provides continuing education for participants and staff on wellness and health related issues
- Performs other duties as assigned
Qualifications :
- Licensed Vocational Nurse (LVN) preferred other licensed medical disciplines will be considered
- Maintains unrestricted license to practice as a Licensed Vocational Nurse (LVN) OR if not an LVN maintains unrestricted license for medical discipline in the state(s) to which assigned
- Maintains current CPR certification
- Demonstrates excellent organizational time management and computer skills
- Demonstrates effective and professional interpersonal verbal and written communication skills
- Able to work independently and as part of a team
