The Bereavement Coordinator provides coordinated care for hospice families of all age groups and provides counseling and referrals to meet the needs of the Hospice family. Coordinates and implements the bereavement component of patient care. This person is responsible for developing and implementing a plan of care for bereavement with the approval of the patients attending physician and the IDT. This person will work with the Hospice team to assure that family members receive the best possible grief and bereavement services. The bereavement coordinator is the Director of Camp Dragonfly and the annual memorial service responsible for the oversight and coordination of all the activities and participants at camp and the memorial service. The Bereavement Coordinator is responsible for the oversight of the bereavement volunteers.
