You will be updated with latest job alerts via emailIn this temporary part-time role youll play a key part in supporting older people leaving hospital to safely transition back home - helping them regain independence and live well where they belong.
This isnt a desk job. Youll be out in the community visiting clients in their homes assessing their functional needs and providing therapy that makes a tangible difference.
Working closely with the multidisciplinary TACP team youll help set care goals deliver interventions and guide clients through short-term reablement with dignity and empathy.
