Empath Health is seeking a Care Team Manager to join our team.
Effectively manages a team of clinical and support colleagues working in an in-patient environment maintaining accountability for providing the highest quality of clinical care and psychosocial support for patients and families.
The Care Team Manager:
- Effectively manages a team of clinical and support colleagues working in an in-patient environment maintaining accountability for providing the highest quality of clinical care and psychosocial support for patients and families.
- Provides positive leadership of a clinical team composed of a variety of disciplinary practices to ensure patient care efficiency effectiveness and satisfaction. Promotes continued professional development.
- Serves as clinical team expert and manages high risk and complex patients/family/staff situations accessing available resources maintaining effective communication and optimizing quality patient care.
- Collaborates and communicates effectively with associate medical director to initiate appropriate level of care on admission and changes in level of care. Co-facilitates Interdisciplinary Group meeting.
- Performs ongoing review of patient documentation to ensure the documentation is complete accurate and timely and to verify that the electronic medical record system (EMR) is being utilized appropriately.
- Maintains responsibility for bed management activity and monitoring inpatient length of stay. Monitors for staffing and census fluctuations and provides direct patient care as needed.
- Ensures compliance with all accrediting agencies State Federal and local government regulations; maintains a strong position against fraud and abuse.
Educational/Professional:
- Must possess current and active Florida licensure as RN; BSN preferred. CHPN a plus.
- Minimum of 2 years recent community-based hospice experience required.
- Recent acute level clinical experience required; GIP care experience preferred.
- Previous management experience and healthcare environment preferred.
- An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
