Job Description

Under the direct supervision of the PACE Home Care Admin Manager the Home Care Coordinator RN is responsible for the development and implementation of homecare services for program participants including the coordination of all Durable Medical Equipment and nursing home care including supervision of Personal Care Assistants (PCA) and Licensed Vocational Nurses (LVN) delivery assistance with participant activities in the center and home environments as assigned.

Schedule is Monday Friday working from 8:00am 5:00pm.
Location: Modesto Center
Compensation: $49.92 $57.79 an hour

Duties and Responsibilities

  • Assesses using the nursing process the home care needs of a frail elderly population and identifies and develops specific plans of care. Initial and periodic assessments to be conducted minimally every six (6) months and must be completed prior to the scheduled team meeting.
  • Communicates participant changes with team members.
  • Coordination of 24 hour care delivery and the implementation of all home care services which include nursing PT OT ST and personal care services to ensure that quality services are provided to meet participant needs.
  • Coordinates and authorizes all Durable Medical Equipment.
  • Coordinates and supervises all nursing care for participants in the nursing home/boarding home according to program standards. Includes interaction with other facility staff.
  • Performs evaluations of HCTs and resource management of HCT staff in conjunction with Home Care Admin Manager.
  • Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning. Communicates any changes in participant condition to team members.
  • Observe each participant for any change in physical mental emotional and social functioning and shall report such changes to the Provider.
  • Ensures all regulations and standards are up to date keeping compliance with orders or directives issued by constituted government/regulatory agencies.
  • Responsible for the provision of home care to meet identified participant outcomes and to achieve the goals as outlined in the participants Plan of Care
  • Ability to interact professionally and respectfully with geriatric individuals including those with cognitive decline and/or physical frailties.
  • Supervise PCAs and LVNs in accordance with regulations.
  • Other projects and duties as assigned.

Physical Demands

  • Ability to lift up to 30 pounds. Moving lifting or transferring patients may involve lifting or pushing greater than 30 pounds should be done with assistance as appropriate.
  • Must be able to hear staff on the phone and those who are served in-person and speak clearly in order to communicate information to clients and staff.
  • Must have vision with or without lenses that is adequate to read memos a computer screen personnel forms and clinical and administrative documents.
  • Must have high manual dexterity.
  • Must be able to reach above the shoulder level to work must be able to bend squat and sit stand stoop crouching reaching kneeling twisting/turning fingering and feeling.

Work Environment

  • Exposure to biohazards including infectious material and waste and any other conditions common in a health care environment.
  • Subject to unpleasant odors.
  • The noise level is usually quiet to moderate but may at times be noisy and crowded.

Education/Experience Requirements

Minimum Qualifications

  • Valid CA Drivers License acceptable driving record and vehicle insurance.
  • Must be able to work effectively in a team environment. Must be able to treat geriatric patients.
  • Only act within the scope of his or her authority to practice.
  • Ability to communicate skillfully handle multiple priorities maintain a warm and friendly demeanor perform nursing duties be flexible innovative focus on results and travel regularly.
  • Meet a standardized set of competencies established by PACE and approved by CMS before working independently.

Education/Experience

  • The Home Care Coordinator RN must have one of the following qualifications:
    • A registered nurse with a baccalaureate or higher degree in nursing or other health related field with three (3) years of experience within the last five (5) years in a home health agency primary care clinic or health facility
    • A registered nurse with four (4) years nursing experience within the last five (5) years in a home health agency primary care clinic or health facility
  • Current unencumbered CA Registered Nurse (RN) License.
  • Current BLS CPR Card certified by American Heart Association.

  • Minimum one (1) year as in a supervisory or administrative capacity

  • Minimum one (1) year working with the frail or elderly population.
