Job Description

Were looking for a licensed Physical Therapist (PT) to support our West LA patients on a per diem basis. This is an excellent opportunity for professionals who value autonomy impact and collaboration.
What You ll Do
Perform evaluations and deliver evidence-based treatment plans tailored to each patient s goals and physical needs.
Provide hands-on care therapeutic exercises and functional mobility training in the home setting.
Collaborate closely with other care providers to ensure continuity and quality across all aspects of patient care.
Offer guidance and education to patients and their caregivers to support long-term recovery and independence.
Maintain clear compliant documentation to track progress and support care planning.
Requirements
What You ll Need
A current Physical Therapist license in California.
At least one year of clinical experience (home health background preferred but not required).
Valid CPR certification.
California driver s license a clean driving history and reliable transportation for home visits.

What You'll Need to Succeed A valid Physical Therapist license in the state of California. CPR certification that s up to date. A current California driver's license, a clean driving record, and dependable transportation for home visits. Availability for a minimum of 25 patient visits per week to be considered full-time. At least two weekdays per week of availability, and for PTAs, two weekend days per month. Bonus Points If You Have: Prior experience in a home health setting (not required, but preferred). A strong commitment to patient-centered care and collaborative teamwork.
