Flexible Scheduling Join Our Growing Home Health Team
Work when you want! Were looking for a compassionate and skilled Physical Therapist to join our expanding Home Health division. If youre passionate about delivering patient-centered care love flexibility and enjoy working one-on-one in the home setting this is your chance to make a meaningful impact.
What You ll Do
Evaluate patients through physical assessments and review of medical history
Develop and implement personalized treatment plans based on each patient s unique goals and conditions
Deliver therapeutic exercises and training to improve strength mobility and function
Educate patients and their families on injury prevention safety and wellness strategies
Monitor patient progress and adjust treatment plans as needed
Collaborate with physicians nurses and other healthcare professionals to ensure comprehensive care
Maintain clear accurate documentation in compliance with home health regulations
Requirements
Master s degree in Physical Therapy
Active state licensure to practice as a Physical Therapist
Minimum of 1 year of experience in home health
Current CPR certification
Valid driver s license and ability to travel to patient homes
Strong clinical judgment and a compassionate adaptable approach to patient care

What You'll Need to Succeed A valid Physical Therapist license in the state of California. CPR certification that s up to date. A current California driver's license, a clean driving record, and dependable transportation for home visits. Availability for a minimum of 25 patient visits per week to be considered full-time. At least two weekdays per week of availability, and for PTAs, two weekend days per month. Bonus Points If You Have: Prior experience in a home health setting (not required, but preferred). A strong commitment to patient-centered care and collaborative teamwork.
