Job Description:
The Registered Nurse (RN) is accountable to the Director of Care Director of Clinical Services and/or Assistant Directors of Care to provide professional nursing care to residents and supervise care given by Personal Support Workers (PSW). Primary functions include assessment and planning of resident care needs; delivery of skilled procedures according to the College of Nurses of Ontario Standards of Practice; medication administration; documentation and resource for Registered Practical Nurses.
RN Charge Nurse is responsible for the building as a Nurse Designate.
The Registered Nurse is required is required to perform their duties in a manner consistent with the Salvation Army Meighen Health Centres core values ensure resident safety and demonstrate customer service excellence while responsible for planning implementation and evaluation of programs and services provided to the residents.
SPECIFIC RESPONSIBILITIES:
- Provides for nursing care in accordance with established policy physicians orders the resident plan of care and within the scope of practice of an RN
- Utilizing sound nursing practice principles the RN will ensure delivery of excellent nursing and personal care on the assigned home area(s)
- Develops and sustains a plan of care to meet residents assessed need
- Responsible for safekeeping of drug and narcotic keys in accordance with professional standards and policies
- Responsible for maintaining accurate and complete records of nursing care delivered utilizing Point Click Care and RAI MDS coding
- Performs RAPs in a timely manner
- Demonstrates a professional working knowledge of all equipment used in the delivery of nursing care
- Follows all corporate policies and procedures; contributes to the development of site specific nursing policies as indicated
- Supports and documents on homes programs in infection control wound care falls prevention restraints minimization and responsive behaviours
- Seeks direction from the ADOC or DOC when resident need exceeds experience or scope of practice
- Liaises monthly with the LIHN Psychogeriatric team; makes referrals; assists the team on their rounds; receives the recommendations and ensures that it is followed by the charge nurses
- Performs post falls assessments as needed
- In charge of the building as a Nurse Designate - attends emergencies such as code red code blue
- Promotes positive work environment and team work by following the Meighen Health Centres policies and procedures
- Puts residents needs first
- Acts as a leader and resource to RPNs
- Financial Accountability:
- Ensures appropriate utilization of all supplies and equipment
- Identifies issues to ensure an adequate inventory of nursing supplies at all times
- Addresses unit supply issues promptly
- Ensures the supply control and safe keeping of drugs and narcotics ensures protocols are followed on all assigned shifts.
- Human Resources:
- Participates in an effective orientation and performance management process in accordance with The Salvation Army Meighen Health Centres standards for all employees within the nursing department;
- Identifies nursing department learning needs and communicates to leadership within the home
- Models positive working relationships with the management and co-workers
- Actively participates in the annual educational program for the nursing department
- Demonstrates relevant understanding of and compliance with legislative knowledge of the Employment Standards Act Human Rights Act collective agreement and MOHLTC Act as it applies to the nursing direct care staff.
- Health and Safety:
- Demonstrates understanding of and compliance with workers responsibilities under the Occupational Health andSafety Act and Regulations WSIB policy and Meighen Health Centres Health and Safety plans policies and procedures
- Attends Health and Safety in-services and taking action to address unsafe conditions and/or procedures to ensure a healthy and safe work environment
- Advises supervisor or designate immediately of all staff incidents that may result in an Occupational Accident Claim any concerns with the physical plant incidents or injuries and completes required reports
- Follows all health and safety policies and procedures. Works safely to reduce the risk of injury to self co-workers and residents
- Wears personal protective equipment as designated
- Maintains a clean safe environment for residents utilizing infection control principles
- Maintains a sound working knowledge of their roles and responsibilities during all emergency situations when working different shifts.
- Community Relations:
- Promotes a positive working environment with the service providers working with MHC
- Establishes and maintains positive communication between the MHC and the community.
- Family and Resident Relations:
- Contributes to positive customer service with residents and families in day to day work
- Conducts ongoing communication with families to inform them of changes in resident health status
- Deals tactfully and courteously with residents residents family visitors and staff in a positive manner
- Facilitates multidisciplinary communication in complex care issues to foster positive resident outcomes
- Establishes and sustains care in a manner that is resident centred and fosters evidence based best practice.
- Quality Management:
- Participates in all aspects of the Quality and Risk management program as it pertains to nursing
- Ensures the collection of clinical indicators within time line on a monthly basis as assigned
- Contributes to the development of recommendations for corrective action
- Maintains personal accountability for completion of monthly audits as assigned.
- Follows established job routines for all personal support workers on a daily basis
- Effectively participates in conferences with the interdisciplinary team providing care to the residents
- Ensures that care provided is documented as required in a timely manner using the assigned documentation tools.
- Ministry of Health Compliance:
- Contributes to good working relationships with relevant persons within the Ministry of Health and Long Term Care (MOHLTC)
- Knows and interprets MOHLTC legislation regulation and process
- Maintains effective liaison with consultants and physicians
- Ensures that systems and processes are in place to achieve and sustain compliance
- Understands all components of related legislation for staff residents and families.
QUALIFICATIONS:
Education & Experience
- Must possess current Certificate of Competence from the College of Nurse of Ontario
- Experience in rehabilitation or long-term nursing preferred
- Proficiency in interpersonal communication skills both written and verbal
- RAI MDS knowledge
- E-mar proficiency
- Proficiency with WORD email internet use and computerized documentation.
Competencies
- Knowledge and familiarity with computer operations and software applications including resident records
- Current knowledge of issues and trends including best practices in the enhancement of quality of life
- Knowledge of current quality improvement and risk management concepts and approaches with special emphasis on the prevention of falls and least restraint policies
- Excellent written and verbal communications skills in English.
Compensation:
The target hiring range for this position is $26.78 to $33.47 with a maximum of $40.16.
Placement in the salary range will be based on factors such as market conditions internal equity and candidate experience skills and qualifications relevant to the role.
