What Youll Do:
As an LPN Home Health and Hospice Nurse youll play a key role in our mission to provide outstanding care. Your duties will include:

  • Delivering skilled nursing care such as wound care catheter care venipuncture and medication management.
  • Educating patients and families on medication compliance and overall health care.
  • Using EMR charting to meet regulatory requirements.
  • Collaborating with a compassionate team including home health and hospice staff providers and families.


What Were Looking For:

  • A licensed LPN in the State of Utah.
  • CPR certification.
  • Strong organizational skills and attention to detail.
  • Ability to build rapport with patients and their families.
  • Problem-solving and critical-thinking skills.
  • Excellent communication skills both verbal and written.
  • A valid Utah drivers license automobile insurance and reliable transportation.


Preferred but Not Required:

  • Experience in home health and/or hospice care.

Job Type:
Part-time / PRN
Compensation: $23.00 - $30.00 per hour
