What Youll Do: As an LPN Home Health and Hospice Nurse youll play a key role in our mission to provide outstanding care. Your duties will include:
- Delivering skilled nursing care such as wound care catheter care venipuncture and medication management.
- Educating patients and families on medication compliance and overall health care.
- Using EMR charting to meet regulatory requirements.
- Collaborating with a compassionate team including home health and hospice staff providers and families.
What Were Looking For:
- A licensed LPN in the State of Utah.
- CPR certification.
- Strong organizational skills and attention to detail.
- Ability to build rapport with patients and their families.
- Problem-solving and critical-thinking skills.
- Excellent communication skills both verbal and written.
- A valid Utah drivers license automobile insurance and reliable transportation.
Preferred but Not Required:
- Experience in home health and/or hospice care.
