The CNS for the Center for Care Management works collaboratively with nurse case managers social workers nursing leaders and the interdisciplinary team to assure patient safety at transition points with a focus on reducing length of stay readmissions and to support throughput. There is opportunity to work with Good Samaritan facilities as partners to improve transitions of care. The CNS will provide education to the team assess and implement evidence based practice and support clinical complex patients throughout the hospital stay and through transitions.

The Clinical Nurse Specialist (CNS) is an expert clinician and advocate in a particular specialty or subspecialty of nursing practice providing leadership in the professional practice setting.

Job Description

Provides expert clinical guidance in a specialized field and coordinates patient care quality initiatives. Monitors the quality patient indicators utilizing evidence-based outcomes and the development and implementation of performance and quality improvement processes as necessary. Assesses a healthcare facilitys nursing practice and its impact on patient outcomes. Uses the results of evaluation analysis to make or recommend process or structural changes including policy procedure or protocol documentation as appropriate to clinical outcomes. Provides consultative care and guidance while serving as a consultant mentor coach researcher and educator to patients families and employees.

Facilitates/collaborates with multi-disciplinary teams that contribute to role development and directly or indirectly advance nursing practice and health services. Develops evaluation strategies to demonstrate patient safety cost effectiveness cost benefit and efficiency factors associated with nursing practice. Identifies expected outcomes that are achievable through implementation of evidence-based practices. Systematically enhances the quality and effectiveness of nursing practice.

Evaluates the practice environment and quality of nursing care rendered in relation to existing evidence identifying opportunities for the generation and use of research. Formally disseminates research findings through activities such as presentation publications consultation and journal clubs. Uses research based data to assist with the development implementation and evaluation for improved clinical practice for all ages developmental needs cultural or psychosocial needs wherever needed. Also uses current healthcare research findings and other evidence to expand clinical knowledge enhance role performance and increase knowledge of professional issues.

Promotes the clinical practice by serving on hospital or community committees locally or nationally. Must be able to incorporate teaching/learning principles and adapt teaching to age groups. Must be able to work with growth and development needs of unique client populations.

Dependent on the market and facility this role may also function in the capacity of an American Diabetes Association (ADA) Coordinator with oversight of the planning implementation and evaluation of the Diabetes Self-Management Education and Support (DSMES) service in order to maintain program standards to obtain recognition.

Qualifications

Successful completion of a masters post-masters or doctorate from a Clinical Nurse Specialist (CNS) program accredited by the Commission on Collegiate Nursing Education (CCNE) or the National League for Nursing Accrediting Commission (NLNAC).

Demonstrated current competence and provision of care treatment or services to an adequate number of patients in the past twelve months or completion of an accredited CNS program in the past twelve months. Experience must correlate to the privileges requested. If/When performing in the role of American Diabetes Association (ADA) Coordinator academic and/or experiential preparation in program management and academic and/or experiential preparation in the care of people with chronic disease.

Certification as a Clinical Nurse Specialist (CNS) by the American Nurses Credentialing Center (ANCC) or an equivalent body as applicable to specialty area of practice. Current licensure by the applicable state. Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances if prescriptive privileges are sought.
