Job Description:

POSITION PURPOSE SUMMARY:

The Pathway of Hope Caseworker provides a high impact case management approach that focuses on identifying and addressing root causes for individuals and families living in poverty. Utilizing a strengths-based approach to interview assess and formulate a plan of action to assist families in moving from crisis to stability. This position provides casework services to individuals and families through the Pathway of Hope (POH) approach.

ACCOUNTABILITIES:

Direct Case Management Services

  • Serve as a contact for Pathway of Hope in the assigned region.
  • Engage and build rapport with the clients.
  • Provide client intake and ongoing assessments to Pathway of Hope participants.
  • Perform all aspects of the case management process using the Pathway of Hope framework (prescreening intake assessment goal planning action transition and follow-up) and utilize Link2Feed as the current Client Management System to record case management actions.
  • Work with Pathway of Hope participants and ministry unit teams to coordinate ongoing goal setting and action plans; schedule regular meetings to review progress and engage in further planning. Effectively maintain records to accurately reflect services rendered using all appropriate forms including accurate documentation and detailed case notes for every interaction in Link2Feed. Ensure confidentiality is maintained in accordance with Operating Policy and applicable legislation. Assist participants in making connections and accessing community resources supporting participants as appropriate. Provide information and referral services as needed to both clients and ineligible participants.

Communication and Reporting

  • Attend training for the sustained delivery of Pathway of Hope.
  • Ensure consistent communication by participating in regular meetings and conference calls with Pathway of Hope teams divisional personnel and the THQ Pathway of Hope Territorial Specialist providing progress reports and requesting any necessary support.
  • Collaborate with the THQ Pathway of Hope Territorial Specialist to promote and organize training opportunities.
  • Attend and participate in trainings and meetings as required by the Ministry Unit Divisional and Territorial Headquarters.

Community Networking and Partnerships

  • Network with other service providers and maintain relationships with community agencies to provide comprehensive services for participants.
  • Attend community meetings as needed.

Health & Safety When Required

  • Ensure that the work area is maintained regarding appearance condition and safety as recommended by Public Health etc.
  • Responsible for the disinfecting of personal high touch areas i.e. desk phone keyboard calculator etc.
  • Required to wear appropriate PPE which may include masks gloves face shields etc. as per Public Health guidelines/recommendations.
  • Ensure the safety and comfort of the ministry unit by maintaining cleanliness and food safety etc. Work in compliance with the OH&S Act and Regulations and abide by The Salvation Armys health and safety policies and procedures Perform other duties as required.

CRITICAL RELATIONSHIP MANAGEMENT:

Internal: Community & Family Services team (staff and volunteers) Corps Officers Congregational members DHQ/THQ and Salvation Army partners External: Community Partners local medical and street outreach services Emergency Services neighbourhood associations POH clientele Government (local provincial federal) resources.

MANAGERIAL/TECHNICAL LEADERSHIP RESPONSIBILITY:

  • This position reports directly to theResource Centre Coordinator.
  • This role works independently (within a team) and does not manage or supervise internal staff but would work across departments to ensure comprehensive services are available to all clientele and may supervise volunteers.
  • Represent The Salvation Army at agency and community meetings as directed.
  • Attend regional Pathway of Hope meetings monthly (in-person or via telephone/video conference).

FINANCIAL AND MATERIALS MANAGEMENT:

Ensure that ministry unit policies and procedures are followed for the safe/secure handling of all monetary donations received and appropriate documentation of gift cards and various vouchers is maintained.

WORKING CONDITIONS:

  • This is a permanent part-time position based on 35 hours per week.
  • Some flexibility in scheduling required some weekend and evening work may be required.
  • The Supervisor will set the hours of work and work schedule.
  • Working environment is typically in the office in generally agreeable conditions.
  • May deal with difficult people and verbal abuse.
  • Ability to lift/move up to 50 lbs.
  • This job requires reaching bending and stooping frequently working in a sitting and standing position for periods of time.
  • Some travel locally and throughout the Kamloops area will be required with possible travel to meetings outside the immediate area for meetings (i.e. DHQ THQ).

Normal hours of work 35 hours per week 8:30 a.m. to 4:00 p.m. includes a % hour unpaid meal break Please note there may be a seasonal temporary increase in hours to a maximum of 40 hours per week for a short period of time during the months of November and December and includes a % hour unpaid meal break as required. Extra hours will include evening and weekend work.

EDUCATION AND EXPERIENCE QUALIFICATIONS:

Education Qualifications and Certifications:

  • A minimum 2-year college diploma in a relevant field and a minimum
  • Alternative combinations of education and experience may be considered

Experience and Skilled Knowledge Requirements

  • A minimum of two (2) years related experience preferably in community engagement or a similar community outreach role
  • Training in de-escalation and intervention strategies/Non Violent Crisis Intervention is an asset.
  • Training (or willingness to be trained) in Naloxone Overdose kit will be required.
  • Demonstrated ability to connect with and establish relationship with those who are marginalized. Hold a valid Class G drivers licence and provide a copy of a drivers abtract for review and verification that is satisfactory to The Salvation Army in its sole discrection. A program vehicle may be used for travel when required

  • Strong organizational and time management skills with the ability to complete assignments and meet deadlines with minimal supervision
  • Current certification in First Aid/CPR or ability to obtain upon hiring
  • Ability and willingness to obtain a valid Safe Food Handlers Certification and Food Banks Canada Food Safety Course.
  • Ability to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army in its sole discretion (i.e. The Salvation Army Abuse Registry) and provide an original copy of a Background Check with vulnerable sector screening as required
  • Ability and willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities

Compensation:

The target hiring range for this position is $21.62 to $27.02 with a maximum of $32.43.

Placement in the salary range will be based on factors such as market conditions internal equity and candidate experience skills and qualifications relevant to the role.
