Hospice RN Patient Care Coordinator:

Location: Starkville MS Department: Operations

At Legacy Hospice we prioritize the quality of life for our patients and strive to create a supportive environment for our staff. As a Hospice RN Patient Care Coordinator at Legacy Hospice youll be responsible for the oversight and provision of patient care by the interdisciplinary team and assisting the Branch Administrator in upholding the overall direction of hospice services.

Key Responsibilities:

  • Evaluate hospice services and personnel using measurable outcomes and objectives.
  • Establish and maintain effective channels of communication.
  • Ensure hospice personnel stay current with clinical information and practices.
  • Conduct continuing education and staff development training.
  • Provide supervisory visits as required.
  • Collaborate with agencies and vendors for effective management of services.

Key Qualifications:

  • Registered nurse in the State with current licensure to practice professional nursing in the State.
  • A bachelors degree in nursing from an accredited program by the National League for Nursing is preferred. Masters degree in hospice preferred.
  • Demonstrated ability to supervise and direct professionals and personnel.
  • Ability to market and deal tactfully with customers and the community.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order.
  • Has excellent observation verbal and written communication skills.
  • Effective social interaction communication and organizational skills and the ability to perform various tasks efficiently.
