City of Greeley: Work Live and Play in Greeley: Summary:
A Community Paramedic performs individually and/or as a member of a team to participate in activities in the following areas: Community health navigation assistance mental health navigation injury/fall prevention; responding to emergency medical services calls and other related public assistance activities; training activities; and performing routine custodial maintenance of fire department equipment apparatus and facilities.
This position also provides integrated health services in partnership with other health professionals and extends access to health services delivery in underserved and general populations. This role extends access to non-urgent health care through in-home patient visits and/or community-based services. This position works in conjunction with an Integrated Case Manager and many other medical and behavioral health providers to develop an individualized plan of care.

Minimum Requirements
One (1)- Three (3) years computer software - ESO Central Square Target Solutions CrewSense Microsoft Word Excel PowerPoint and other software applications as necessary.
High School Diploma or GED
Valid Drivers License
State of Colorado Paramedic
Basic Life support Certification
Advanced Cardiac Life Support Certification
Pediatric Advanced Life Support Certification
Preferred
State of Colorado Community Paramedic (P-CP)
Or State of Colorado Registered Nurse
One (1) - Three (3) years clinical experience in healthcare setting such as clinic emergency room/department physicians office
5 years of paramedic experience in a 911 system
NREMTP
Associate or Bachelors degree preferred
Knowledge Skills and Abilities
- Knowledge of complicated practices and precedents. Displays basic understanding and application of theoretical or scientific principles acquired through college education or advanced vocational skills strengthened by years of on-the- job experience. The nature of the tasks typically requires a specialized field of knowledge (e.g. engineering accounting health sciences education computer systems human resources etc.)
- Withholds and maintains a current Colorado or National Registry Paramedic certification.
- Advanced understanding and knowledge of human anatomy and physiology.
- Knowledge and experience of Fire Rescue and Emergency Medical equipment. Including not limited to: Fire Apparatus and Staff Vehicles.
- Knowledge of computer software - ESO Central Square Target Solutions CrewSense Microsoft Word Excel PowerPoint and other software applications as necessary.
- Ability to focus on activities that have the greatest impact on meeting work commitments.
- Ability to establish and maintain partnerships with a variety of internal and external constituencies.
- Ability to communicate and work effectively with individuals from diverse backgrounds cultures and ages.
Essential Functions:
- Provides community health navigation assistance mental health navigation and injury/fall prevention.
- Performs routine custodial maintenance of fire department equipment apparatus and facilities.
- Responds to emergency medical services calls and other related public assistance activities.
- Works within policies practices medical protocols and SOPs developed for safe & effective operation in EMS response.
- Emergency Response Readiness - Training - Participates in regular training activities related to Community Health and Emergency Medical responsibilities.
- Emergency & Non-Emergency Response - Performs emergency medical services activities at the paramedic level of care including driving fire apparatus operating related equipment provides prevention and diversion activities and administers medical aid at their medical certification level.
- Public Education and Fire Prevention Activities - Presents information to the community on safety medical and fire prevention topics. Participates in community collaborative meetings and care planning with community partners.
- Administrative Duties - Completes timely and accurate reports using computer software. Familiar with proper documentation procedures.
- Communication - Common mission public events etc. Communicates as part of a team response training etc. Collaborates on response issues resource alignment etc. with local regional and national fire departments. Continuity of patient care communications scene safety coordinates response etc. with ambulance transport contractors emergency room staff dispatch center coordination etc. Treats patients answers the general phone line greets visitors attends meetings provides public safety education fire prevention topics etc.
- Problem solving and decision making- Solves a variety of problems in situations where there are substantially diversified procedures specialized standards and only limited standardization. Changing priorities or differing situations require the incumbent to consider which among many procedures should be followed in what sequence to achieve the required job results. Requires reasoning to produce a practical course of action within the established process. Choices fall within the range of specified acceptable standards alternatives and technical practices.

- Approximately 80% travel is required

- This position operates under the direct supervision of a Fire Lieutenant in the non-emergency and emergency setting.
- Work is primarily performed in the community.
- Incumbent performs work requiring strength and agility during training assigned duties and during emergency response in all kinds of weather while wearing protective equipment.
- Responds to medical situations where incumbent may be exposed to toxic and hazardous environments and extreme weather conditions.
Physical Requirements:
- Moderate to considerable physical effort that includes carrying common medical equipment and supplies lifting patients from ground and other physical exertion that could exceed 50lbs may be required in this role.
- Incumbent performs work requiring strength and agility during training assigned duties and during emergency response in all kinds of weather while wearing protective equipment.
HAZARDS
- Dangerous to considerable hazards reflect a frequent exposure to conditions that are unpredictable and may lead to injury
- Responds to medical situations where incumbent may be exposed to toxic and hazardous environments and extreme weather conditions.
