Job Duties in brief:
LPNs process referrals by assessing the information with attention to the CA Checklist and the requested information. Collaborate with referring stakeholders. Ensure referral accuracy adequacy and completeness by requesting all missing documentation necessary for medical clearance. LPNs will review the referral and determine if the patient meets criteria for the Short-Term Care Facilities (STCF) Diversion bed or County hospital based on Insurance and psychiatric presentation.
