This person in this role will be expected to provide care in various residential settings including private homes assisted living and skilled nursing facilities.

Job Summary:
The Hospice Admission Nurse is responsible for conducting comprehensive assessments of patients referred for hospice services. This role involves evaluating patients medical histories coordinating care with the interdisciplinary team and facilitating a smooth admission process. The nurse will also provide education and support to patients and their families regarding hospice services.
Primary Responsibilities:
- Conduct comprehensive assessments of patients referred for hospice addressing physical emotional and spiritual needs.
- Collaborate with physicians and the interdisciplinary team to develop and adjust individualized care plans.
- Educate and support patients and families during the hospice admission process ensuring understanding and compassion.
- Complete required documentation accurately and promptly.
- Ensure all medical and administrative admission requirements are met.
- Participate in team meetings case conferences and ongoing training.
- Perform regular hospice visits including death visits and participate in the on-call rotation.
- Stay current with hospice best practices policies and regulatory standards.
- Perform other related duties as assigned.
Qualifications:
- Education & License: Active RN license required; hospice/palliative care certification (e.g. CHPN) a plus.
- Experience in hospice or palliative care settings is preferred along with a strong understanding of end-of-life care principles and best practices.
- Strong clinical assessment communication and interpersonal skills with a compassionate patient-centered approach.
- Certifications: BLS required; ACLS preferred (based on care setting).
- Technical Proficiency: Proficient with EHR systems and healthcare technologies.
- Other Requirements: Ability to work independently respond to crises and adapt to diverse social cultural and religious backgrounds. Must have reliable transportation a valid drivers license and auto liability insurance.

Compensation: $104000 - $118000 per year depending on experience.
All rates are negotiable within this range and will be discussed during the hiring process.
