Job Title: Family Nurse Practitioner
Department: Clinical
FLSA Status: Exempt
Schedule: Full-Time (40 hours/week)
Reports To: Practice Manager (Operationally) Chief Medical Officer (Clinically)

We are seeking a dedicated and experienced Family Nurse Practitioner (FNP) to join our integrated care team. In this role the FNP will provide advanced patient-centered primary and specialty care with a focus on prevention wellness and chronic disease management. The successful candidate will be expected to maintain an efficient patient load of at least two patients per hour during clinical hours while promoting the values and mission of our organization.
As a key healthcare provider in a federally designated rural and medically underserved community you will play a critical role in delivering high-quality equitable care to individuals and families across the lifespan.
Essential Duties and Responsibilities:Perform comprehensive physical exams and clinical assessments
Document detailed medical histories symptoms and patient concerns
Develop and implement individualized patient care plans
Order interpret and follow up on diagnostic tests and imaging
Diagnose and treat acute and chronic health conditions
Prescribe medications and coordinate treatment regimens
Modify care plans based on patient progress and changing needs
Provide education and counseling on wellness disease prevention and self-care
Collaborate with physicians specialists and other healthcare professionals
Instruct patients and families in managing health conditions and promoting healthy lifestyles
Operate within established clinical protocols and quality care standards
Masters degree from an accredited nurse practitioner program
Current licensure as a Nurse Practitioner in the State of Massachusetts
Board certification through ANCC or AANP
Prescriptive authority in Massachusetts
Minimum of 1 2 years of experience providing primary care as an FNP
Strong knowledge of HIPAA OSHA and other applicable healthcare regulations
Proficiency in electronic medical records (EMR) and point-of-care documentation
Excellent clinical judgment communication and time management skills
