Pay Range: $81400.00- $122100.00/ year (posted pay range is based on a 1.0 FTE)

POSITION SUMMARY:
The Manager of Care Management has well developed skills in the assessment and acute care management of patients and families. The scope of practice includes assessment and management of high-risk high-cost patients multidisciplinary team leadership program development clinical outcomes measurement clinical and fiscal variance analysis. Offers clinical consultation to all staff and provides case management services within the area of responsibility for a select number of patients who meet high-risk screen indicators.
The Manager of Care Management has the authority and responsibility for providing leadership and supervision to utilization management staff care managers care coordinators and social workers for both inpatient and outpatient services. Participates in continuous quality improvement activities and educational experiences in support of departmental and divisional nursing philosophy and objectives as well as Health System initiatives. Plans and participates in community outreach activities.
EDUCATION/TRAINING:
- Bachelors degree in Nursing required.
LICENSURE/CERTIFICATION:
- Current licensure in the State of Illinois as an RN.
- Must obtain certification in specialty within 2 year of hire. Acceptable specialty certifications are Certified Case Manager (CCM) or Accredited Case Manager (ACM).

MAJOR RESPONSIBILITIES:
- Oversees case management staff and ongoing development of this function throughout the System. In conjunction with physicians nursing staff and other health care professionals works to improve the quality of patient care and ensure the efficient and cost effective movement of patients through the health care continuum.
- In conjunction with Director formulates department goals objectives standards policy and procedures for quality and patient safety efforts and initiatives throughout the system. Develops and manages operating and capital budgets ensuring positive financial outcomes.
- Develops and implements annual performance goals that reflect the organizations strategic plan and values and assures compliance with accrediting regulatory and safety standards. Integrates current best practices for clinical resource utilization into its policies and procedures.
- As a coach and mentor assesses skill level of all staff facilitating their growth and development while maintaining an environment that promotes positive change and advances the CQI philosophy. Responsible for monitoring staff performance recommending corrective action and appropriate discipline completing performance appraisals and ensuring compliance with department and hospital rules and regulations.
- Plans and implements Joint Commission and Conditions of Participation Compliance for Utilization Management and Care coordination as appropriate. Develops communicates and implements quality measurements improvement processes and standards within the Health System facilitating multidisciplinary collaboration.
- Screens all assigned unit cases based on social work team established criteria and actively pursues identified needs for patients and their families. Provides counseling to assist patients and their families in coping with problems related to illness and/or disability.
- In collaboration with medical nursing ancillary and support staffs maintains the nurse case manager and social work case manager roles within the System. Facilitates effective relationships with resources within the community ensuring effective care across the continuum of care.
- Attends or initiates patient-centered conferences and complex care rounds on assigned units to discuss key social financial and discharge planning factors which may affect patients responses to treatment. Educates patients and patient families regarding the effects of illness and disability on personal adjustment family dynamics relationships and social issues.
- Attends and participates in all required meetings including interdisciplinary treatment planning administrative in-service training etc. Remains informed of agencies professional organizations and other resources that will assist patients and families in providing continuity of care after discharge and directs patients and patient families to same as appropriate.
- Collaborates with community partners to assure smooth transitions of care for the health system patients.
- Maintains knowledge of current trends and developments through journals professional associations and related seminars. Maintains Continuing Education Units as required by license or certification.
