You will be updated with latest job alerts via emailAre you a compassionate and skilled LPN eager to make a difference in community-based care Join a dedicated team providing personalized nursing services in Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) settings supporting adults who need extra help to live safely and comfortably at home.
Key Responsibilities:
Provide skilled nursing care tailored to the unique needs of AFC and GAFC residents including medication administration wound care and monitoring chronic conditions.
Conduct comprehensive health assessments and complete MDS-HC (Minimum Data Set - Home Care) evaluations to capture resident health status and needs.
Document resident status thoroughly and report any changes promptly to supervising RNs and healthcare providers.
Collaborate with multidisciplinary teams to develop implement and update individualized care plans.
Supervise and support Caregiversand other direct care staff to ensure high-quality care delivery.
Ensure compliance with Massachusetts Department of Public Health regulations and AFC/GAFC program standards.
Facilitate health education for residents and caregivers promoting wellness and independence.
Participate in emergency preparedness and safety protocols within the qualified homesetting.
Maintain accurate and timely clinical documentation including monthly nursing assessments progress notes and MDS-HC reporting.
Qualifications:
Current and valid LPN license in Massachusetts.
Experience in community-based care preferably in AFC/GAFC or similar long-term care settings.
Proficient in conducting MDS-HC assessments and using findings to guide care planning.
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills to work effectively with residents families and healthcare teams.
Knowledge of regulatory requirements for AFC and GAFC programs.
Ability to provide compassionate care respecting the dignity and preferences of each individual.
