POSITION SUMMARY

* Flexibility in scheduling with ability to work evenings and/or weekends.

Consistent with the interprofessional setting and the mission vision and strategic directions of CHEO the Social Worker provides intake assessments short-term therapeutic intervention clinical back and group facilitation to youth who are taking part in our Outpatient Mental Health Services primarily through Youth Net-Reseau Ado (YN/RA) and the Transitional Youth Program. YN/RA supports youth 13 20 years of age in a youth friendly bilingual by youth for youth mental health promotion program which offers alternative support services for youth with an aim to reduce the stigma surrounding mental health & mental illness through activities education research and advocacy. The Transitional Age Youth Program addresses the mental health needs of individuals 16 24 years of age. This program offers group-based interventions and helps to bridge the gap for emerging adults in our community. The Social Worker will conduct intake appointments and will have an active role in program development service evaluation education and skills-based training as required. The Social Worker will participate in providing mentoring and training opportunities to Social Work students studying at the BSW/MSW level.

Please note that the initial assignment will be supporting Youth Net-Reseau Ado (YN-RA) and the Transitional Age Youth Program (TAY); however the time split may be variable dependent on needs.

The Social Worker will be a member of the CHEO Discipline of Social Work and may be required to participate in the Social Work Coverage Model.

MAIN RESPONSIBILITIES

  • Conducts intake assessments in a youth friendly manner by methods such as interviewing youth obtaining relevant information and gathering social data in accordance with professional practice standards and clinical policies.
  • Utilize the Electronic Medical Record (EPIC) as required
  • Provides short-term counseling (1 8 sessions) crisis intervention and clinical support to group facilitators and conflict resolution for youth participating in this service.
  • Engages in clinical on-site support for intervention programs such as the monthly drop-in and weekly programming
  • Provides support to the Youth Advisory Committee (YAC)
  • Participates in facilitator training (clinical protocol and support) and community presentations
  • Provides group based interventions and co-facilitates with mental health community provider
  • Develops relationship with adult mental health services and builds partnerships
  • Travel to different sites including schools and community locations
  • Engages in training and the delivery of education to Social Work students and health care professionals
  • Meets youth in the community or on site
  • Maintains current knowledge on social work evidence based practice by such methods as reviewing literature consulting with clinical and community experts in the field evaluating clinical practices and participating in professional development activities in order to improve care through new practices.
  • Performs administrative tasks such as maintaining clinical documentation progress notes workload statistics and updating health records in accordance with established standards.
  • Participates in interdisciplinary team meetings.
  • Liaises with various community partners including the School Boards child welfare organizations and community mental health providers.
  • Participates in and supports clinical research activities with the inter-professional team members.
  • Participates in committees within the discipline of Social Work
  • Other responsibilities as assigned.

QUALIFICATIONS

  • M.S.W. graduate from an accredited university program (Preferred)
  • B.S.W. graduate will be considered if other qualifications are met (Essential)
  • Minimum 3 years experience working in youth mental health (Preferred)
  • Bilingual (French and English) (Essential)
  • Flexible work schedule and availability to work evenings as required (Essential)
  • Experience in risk assessment and crisis management (Essential)
  • Knowledge and experience working with addictions (substance abuse and gambling) (Essential)
  • Well-developed group work skills (Essential)
  • Strong interpersonal and teamwork skills (Essential)
  • Youth-friendly and knowledge of specific needs of emerging adults (Essential)

Conformment au contexte interprofessionnel et la mission la vision et aux orientations stratgiques du CHEO social.e fournit des valuations de ladmission une intervention thrapeutique court terme du soutien clinique et des services danimation de groupe aux jeunes qui participent nos services externes en sant mentale principalement par lintermdiaire de Youth Net/Rseau Ado (YN/RA) et du programme pour les jeunes en ge de transition (Transitional Youth Program). YN/RA aide les jeunes de 13 20 ans dans le cadre dun programme bilingue de promotion de la sant mentale par les jeunes pour les jeunes qui offre aux jeunes des services de soutien alternatifs visant rduire la stigmatisation entourant la sant mentale et la maladie mentale par des activits la sensibilisation la recherche et la dfense des intrts. Le programme pour les jeunes en ge de transition rpond aux besoins en sant mentale des personnes ges de 16 24 ans.
