Schedule: Part Time: Mon-Friday schedule

Job Overview:

A busy Cardiology Office at Bristol Health Multispecialty Group is seeking a Registered Nurse - Cardiology with the clinical expertise to manage calls from our patients with the goal of providing patient-centered high quality care. By answering patient calls in a timely manner and following through to resolution of patient concerns you will be an integral part of BHMGs goal to provide an outstanding patient experience. Low call abandonment rates one-call resolution and increased patient satisfaction are clear indications of success in this role.

Main Responsibilities and Duties:

  • In collaboration with the physicians/advanced practice professionals (APPs) develop implement and manage plans of care
  • Instruct patients and families on proper treatment health maintenance and disease prevention over the phone via email and face-to-face
  • Employ critical thinking and clinical expertise to scan the patients medical history laboratory results and medications for information germane to the patients concern
  • Utilize critical thinking and clinical knowledge to ask relevant questions to accurately identify the patients concern and identify the most important details vital to the triage process
  • Follow protocol-driven triage guidelines
  • Rapidly identify emergency situations and refer to 911
  • Provide instruction on parameters necessitating call back
  • Accurately prioritize based on nature and severity of illness/injury/concern
  • Assist with care coordination by closing identified gaps in care for the practices population
  • Assist patients/callers in navigating through the healthcare system
  • Provide support and answer questions about patients ongoing care
  • Effectively employ Teach-Back techniques to confirm understanding for patient safety
  • Enter detailed documentation of all calls in the EHR at the time of the call
  • Assist with medication refills as designated by the physician/APP
  • Monitor laboratory and diagnostic results to ensure they are performed resulted and reviewed by the physician/APP and that the patient has been notified of the results and any follow up recommended
  • Obtain prior authorizations for medications diagnostic tests and procedures as indicated
  • Follow through on calls to completion (closed-loop communication)
  • Answer incoming calls from patients in a timely manner
  • Check voicemails regularly and ensure all messages are taken off voicemail before end of shift
  • Schedule patient appointments when needed
  • Assist with rooming procedures medication/immunization administration as needed

Qualifications

Educational / Job Qualifications:

  • Must have a valid State of Connecticut Registered Nurse license
  • Must have valid CPR certification.
  • Must have a minimum of 2 years of clinical experience in a cardiology setting.

  • Experience with electronic medical records.

Bristol Hospital Mandated Educational Requirements

General orientation at time of hire. Fire/Safety/Infection Control annually. Other programs as mandated by Hospital.

Special Requirements:

Demonstrated proficiency in electrocardiographic testing. Must be able to take vital signs knowledgeable in medical terminology anatomy and physiology. Familiarity with medical equipment common to a medical practice.

Physical Requirements:

Bending stooping walking sitting on a daily basis. On feet most of the day. Assists patients in and out of treatment rooms in and out of chairs on and off exam tables. Ability to spend one to four hours/day at computer terminal/personal computer performing data entry analysis. Must be able to hear with sufficient acuity so as to understand routine conversation in person or over telephone. Must possess sufficient visual acuity so as to read use computers and conduct essential job functions in an acceptable manner. Must be able to speak so as to be understood by the average person. Significant documentation and paperwork review requiring extensive visual acuity.

Work Environment:

Medical office setting with direct exposure to patients body fluids possible exposure to blood borne pathogens. High volume of telephone and personal contact with customers.

Cognitive Requirements:

Must be able to triage patients according to need and time requirements. Must possess excellent communication skills. Must provide comprehensive accurate initial assessment to Medical Providers. Must be able to read both routine and technical information with comprehension. Must be able to write so the average person can comprehend. Must be able to perform simple calculations for medication doses physical exams and tests administered.
