Job Description
Location: Mission Viejo CA
Duration: 13 weeks
Shift: 8:00 AM 4:30 PM (5x8-Hour)
Pay Rate: $76/hr (W2) $81/hr (1099)
In this role the LCSW will assess plan and provide appropriate interventions and support services for patients undergoing cancer treatment.
- Perform biopsychosocial assessments and develop individualized care plans
- Provide emotional support individual counseling and resource referrals to patients and caregivers
- Coordinate care and collaborate with internal and external healthcare providers
- Assist patients in navigating community and financial support resources (e.g. HMOs housing charities transportation medication assistance)
- Document patient interactions and responses according to hospital standards using Epic
- Offer education and guidance regarding advance directives and patient rights
- Monitor and respond to patients with complex medical and psychosocial needs particularly related to oncology diagnoses
- Department: Social Work (Oncology)
- Patient Population: Adults undergoing cancer treatment
- Staffing Ratios: Day 1:5
- Active LCSW license in California
- Minimum 5 years of LCSW experience (3 years accepted if in oncology setting)
- Proficiency with Epic charting system
- Demonstrated knowledge of adult protective services hospice ADA regulations communication disorders and major disease processes (e.g. cancer immune deficiencies neurological disorders stroke)
- Case management and care coordination
- Individual counseling and crisis intervention
- Familiarity with outpatient clinic settings and physician office workflows
- Knowledge of community resources for patient assistance and support
