Job Description

In this essential position you will provide comprehensive nursing care to patients confronting terminal illnesses ensuring they maintain the highest possible quality of life. You will work closely with families and interdisciplinary teams offering both emotional and physical support throughout the patients journey. Our approach to end-of-life care is holistic focusing on honoring the wishes and values of each individual through personalized care plans.

Key Responsibilities:

  • Manage the care of patients nearing the end of life ensuring their comfort and dignity.
  • Schedule patient visits based on assessed needs and collaborate with physicians to ensure cohesive care.
  • Oversee the management of medication and patient orders.
  • Supervise Licensed Practical Nurses (LPNs) Licensed Vocational Nurses (LVNs) and Hospice Aides according to established protocols.
  • Collaborate with a team of medical professionals to deliver high-quality palliative care addressing physical emotional and spiritual needs.

Qualifications:

  • Associates degree preferred; Registered Nurse license required.
  • Minimum of one year of nursing experience with previous hospice experience preferred.
  • Willingness to serve on-call as needed.
  • Must possess reliable transportation and a valid drivers license.
