Pediatric RN Case Manager
Up to $5000 sign on bonus $$$$$
Location: Bridgeport/New Haven Area
Schedule: MondayFriday Day Hours

What Youll Do as a Pediatric RN Case Manager:
Coordinate Pediatric Home Care Oversee individualized care plans for children with acute and chronic conditions
Deliver Direct Nursing Care Administer meds manage wounds trachs vents and respond to clinical needs
Educate & Empower Families Provide instruction and emotional support to caregivers and families
Collaborate with a Care Team Work closely with physicians therapists and internal staff to ensure quality outcomes
Monitor Document & Adjust Track progress respond to changes in condition and maintain clear clinical records
Lead with Compassion Serve as an advocate and champion for your patients and their families
What Youll Need:
Graduate of an accredited nursing program
At least 1 year of RN experience with pediatric care strongly preferred
Active RN license in the state of Connecticut
Reliable transportation valid drivers license and auto insurance
Ability to lift up to 100 lbs. and perform physical nursing tasks
Strong clinical judgment communication skills and a heart for service
