RN - IHH Berryhill Center
UnityPoint Health - Fort Dodge
FTE: 1.0 (Full-time 40 hours per week)
Shift: Monday-Friday 8a-4:30pm

Responsibilities
Assess patients medical and behavioral health needs
Review all patient medications and check for potential drug interactions
Ensure that the patient all providers who prescribe medications for the patient have an accurate medication list and holistic treatment plans
Refer patients to community resources
Create an integrated health treatment plan with patient engagement
Follow up with referral sources to ensure that patients and other providers are following their treatment plan
Obtain health information from patients including but not limited to height weight waist circumference allergies current and previous medications and smoking
Responsible for ensuring that ones caseload is meeting health performance indicators
Enter patient health information into computer system
Assist patients in completing patient assistance and other public assistance forms
Assist patients in completing Integrated Health Home enrollment forms
Assist patients in completing health questioners
Provide direct services to patients as needed including but not limited to transportation
Assist patients in completing quality assurance surveys
Day travel to satellite clinics may be required. Mileage for travel will be paid
Qualifications
REQUIRED QUALIFICATIONS:
License RN or higher
Six months experience working in a medical clinic home health hospital or nursing home as an employee or intern.
A valid drivers license auto insurance and reliable transportation
Computer Skills
PREFERRED QUALIFICATIONS:
Experience providing behavioral health services
