Qualifications
Education
- Minimum Level of Education Required: Associates Degree (required) Bachelors Degree (preferred)
- Area of study or major: Early Childhood Teaching Social Work or Nursing degrees preferred or related field preferred not required.
Licensure/Certifications
- Valid Nursing teaching or social work license from the state of Ohio (preferred)
Experience
- Minimum two years experience with young children and their families if degree is not early childhood teaching social work or nursing. Demonstrates a thorough knowledge of and the ability to work with community resources and serve the needs of at-risk populations and communities.
- At least 1 year working with pregnant women infants and children (preferred).
- Upon hire complete training and certification per the Help Me Grow guidelines.

Requirements Include:
- Possession of a valid drivers license reliable transportation and current auto insurance.
- Must adhere to agency policies regarding tobacco use including maintaining a smoke-free presence during work hours and ensuring that clothing and personal items are free from the smell of tobacco.
- Position specific testing requirement: Must undergo a criminal background check with the Ohio Bureau of Criminal Investigation and a background check with the OhiosStatewide Automated Child Welfare Information System(SACWIS) Registry on Child Abuse and Neglect.
- Working in the community potentially under stressful conditions while managing a caseload of families.
- Driving daily to client homes and community sites in own vehicle occasionally in adverse weather conditions.

- Standard Business Hours are Monday -Friday 8:00a.m-4:30p.m. Extended Business Hours to meet the needs of our clients are Monday- Friday 8:00-6:00 p.m. and Saturday 8:00 a.m. -4:00 p.m. Extended Business Hours are solely for the purpose of accommodating a clients work or school schedule.
- If working Extended Business Hours employees should flex their time throughout the week to avoid working more than 40 hours each week.
Essential Duties & Functions
- Deliver structured evidence-based curriculum during recurring home visits to promote school readiness parent-child interaction healthy development and overall family wellbeing.
- Adapt lesson plans and materials based on the familys goals cultural background and childs developmental level while maintaining fidelity to all applicable program requirements and guidelines.
- Establish and maintain supportive relationships with families to build trust and encourage active participation in the program.
- Assess family strengths and needs through interviews screenings and collaborative goal setting.
- Develop and monitor service plans that address goals related to education employment health housing and child development.
- Provide referrals and coordinate access to community resources such as healthcare mental health food assistance housing and employment services. Document all contacts assessments referrals and follow-ups in an accurate and timely manner in accordance with program policies and confidentiality standards.
- Collaborate with internal teams and external stakeholders to support coordinated care.
- Support families through transitions including entry to preschool childcare employment or other programs.
- Promote school readiness by supporting parent-child interactions and early learning activities during home visits.
- Maintain compliance with all program standards policies and procedures including mandated reporting laws and documentation requirements.
- Participate in team meetings supervision and professional development to enhance service quality and stay informed of best practices.
- Engage families in group activities or workshops when applicable including parenting classes health education and family engagement events.
- Collaborate with leadership to identify learning needs and formulate self -development goals/developmental plan.
- Assist with mentoring of home visitors/service coordinators.
- Attend community meetings and events as set forth by leadership.
