Key Responsibilities
- Provide management and delivery of patient care plans
- Collaborate with the care team
- Consistently meet critical deadlines for charting
- Self-manage time and schedule efficiently
- Schedule (Including any on call/weekends): M-F 8am-5pm; full time; 1 weekend/month; 2 holidays/year; no on call
- Local travel to residential homes
- Bachelors degree or higher
- Healthcare (1 years)
- 1 years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- TruChart EHR
- Valid Physical Therapist in Pennsylvania (PA)
