Provides leadership and oversight guiding strategic development planning implementation evaluation and continuous improvement within the inpatient care management space. Ensures seamless transitions between inpatient care management outpatient case management post-acute care networks and affiliate hospitals. Collaborates with various stakeholders including hospital and health plan leaders administrative leaders inpatient and ambulatory care teams pharmacy post-acute care providers revenue cycle leaders and legal and regulatory departments. Maintains high standards of care and monitoring data metrics to achieve organizational goals and objectives.Job Duties:
