Assesses monitors detects plans and takes appropriate action to prevent potential physiological complications associated with wound ostomy or continence. Provides information and instruction to patients and their caregivers. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community. Participates in projects related to area of Wound Ostomy and Continence Nursing and developing and evaluating educational materials and programs and policies.
Essential Functions
Assesses and monitors the overall physical needs of the patients/family in regard to ostomy wound and continence. Evaluates each individual patients response to treatment.
Assists in developing/maintaining an individual plan of care with the patient consistent with changing needs and goals reflecting full scope of practice.
Collaborates in discharge planning for patient at time of patient contact including families caregivers as appropriate. May facilitate a smooth transition to the next point of service/home.
Assesses the educational needs of a select group of patients caregivers and staff. Develops and implements appropriate teaching strategies and makes appropriate referrals for patients providing counsel to enable them to adjust to living with a specific medical condition. Documents nursing care/teaching consistent with documentation guidelines. Ensures that required documentation for activities within area of responsibility is performed in an accurate and timely manner.
Collaborates with nurses providers and other departments to develop implement and evaluate programs and services that support patient care delivery in Wound Ostomy and Continence Nursing. This includes effective utilization of workforce equipment and supplies. Develops interdisciplinary relationships to support improvements in care delivery and patient staff and provider satisfaction.
Collaborates with nursing quality department to develop and maintain quality monitoring system to continuously monitor and measure the quality safety and outcomes of nursing services for assigned area. Develops and implements unit quality indicators to evaluate performance and the level of practice for Wound Ostomy and Continence nursing as well as develops/implements improvement plans to correct variances.
Participates in the development and review of policies procedures and guidelines related to wound ostomy and continence based on evidence-based practice research findings and Spectrum Health quality outcomes.
Encourages an environment supportive of nursing research promoting an evidenced-based clinically effective nurse-sensitive patient outcomes for Wound Ostomy and Continence Nursing.
