Job Description
Overview
Join our clients extraordinary care team as a Hospice LPN in Glen Carbon IL. This full-time position offers a rewarding opportunity to provide outstanding care in a team-based environment. You will collaborate with colleagues to deliver patient care plans and support patients in the local area. Key Responsibilities
- Collaborate to provide management and delivery of patient care plans
- Support patients in Shiloh/Fairview Heights and surrounding areas
- Meet critical deadlines for charting
- Self-manage time and schedule
- Drive to/from patient locations
- Work schedule and shift expectations will be provided during onboarding.
- Occasional local travel within the specified territory
Qualifications
- Associate degree or higher
- Hospice (2 years)
- 1 years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- TruChart EHR
- Valid Licensed Practical Nurse (LPN) in Illinois (IL)
- Do you have a valid Drivers License Reliable Mode of Transportation and Auto Insurance (all 3)
- Are you okay with the required travel for this position
