Compensation
- $68000 -$70000/year
We are looking for a compassionate LPN to join our growing team! We are a family-owned company. The right candidate has a can-do attitude excellent interpersonal skills and a current state LPN license. In this role you will be providing medical support to our clients while ensuring a safe environment for them.

Responsibilities
- Assess residents in home hospital rehab and other settings to determine initial care plans and appropriateness for move-in
- Complete re-assessments at 30 and 90 days as well as when there are significant changes and conduct care conferences with residents and families to discuss care changes.
- Manage all Resident Care Staff including completing education discipline and performance reviews.
- Communicate with ancillary staff and coordinate continuation of care services.
- Oversee the Infection Prevention and Control program and lead quarterly meetings.
- Oversee Incident Management and Quality Review Program
- Manage all aspects of resident care to ensure that residents are well cared for and treated with dignity and respect.
Qualifications
- 1 year experience as an LPN in a home care setting
- Current state license as an LPN/RN
- Proof of current CPR certification
- Ability to work flexible schedule.
- Excellent interpersonal skills
- Valid drivers license and reliable transportation
