About the position:
The Admissions & Coordination Work Lead oversees and supports the admissions and service coordination process within Mental Health Transitional Living (MHTL) Homes programs overseen by OCFMH. This role ensures efficient client intake proper service matching and compliance with policies and regulations. They provide leadership to the admissions team resolve complex cases and improve workflows to enhance service delivery. The Work Lead also manages scheduling challenges staff training and client concerns from referral to admission ensuring a seamless and supportive experience for both clients and providers. This position is also responsible for interpreting and ensuring compliance with all applicable laws rules standards and regulations; oversight of the admissions and coordination policies and procedures for the MHTL Homes. This position acts as a subject matter expert in admissions and coordination ensuring that team members deliver accurate timely and consistent services as it relates to the MHTL Homes admissions and coordination activities. Duties in this position include:
Oversight of Admissions Processes:
ensures that the admissions process runs smoothly by managing workflows reviewing documentation and ensuring compliance with policies and regulations at all of the Mental Health Transitional Living Homes. This includes continuous review to ensure the admissions processes and workflow are efficient fair and uphold the integrity of the programs
provides training and support to admissions staff
creates tools and resources such as an admissions checklist electronic tracking system to track admissions etc. to ensure the admissions process is efficient and effective
supports the coordination of client referral review meets regularly with the admissions and coordination team to review referrals and makes data-informed decisions to enhance both program effectiveness and client outcomes
plays a critical role in identifying and implementing necessary changes to improve admissions procedures while upholding the programs mission and values
provides guidance and support to admissions and coordination staff ensuring they have the resources and training needed to perform their duties effectively
coaches and mentors staff to ensure a cohesive and professional work environment
provides feedback to staff and deals with any staff conflict at the initial stages of the conflict
maintains open communication and regular meetings with staff to ensure they have all resources and tools to perform their jobs
ensures that all admissions and coordination processes comply with internal policies and external regulations including HIPAA state licensing requirements and funding guidelines
position creates and updates policies relating to the admissions processes and procedures
creates and provides training to staff relating to admissions processes and changes
responsible for addressing cases that require special attention such as clients with complex needs legal issues or urgent situations requiring immediate coordination
