Job Purpose
- Working as an Occupational Therapist in the Countywide review team your role will involve working with adults from 18 years and upwards with physical disabilities learning disabilities acquired brain injuries and chronic health conditions. The focus is to complete a thorough strengths-based review including mental capacity act assessments and Continuing Health Care checklists and decision support tools.
Key Duties/Accountabilities
- Work as part of a strong and dynamic reviewing service contributing to strengths-based assessments utilizing Occupational Therapy skills and knowledge.
- Develop and implement a plan of support aimed at improving wellbeing and delivering outcomes which may include advice information the provision of assistive equipment and recommendations for home adaptation.
- Use up-to-date evidence-based practice to inform interventions carry out risk assessments and undertake assessments under the appropriate adults legislation.
- Contribute to Continuing Health Care assessments utilizing specialist knowledge to input into this Multi-Disciplinary assessment.
Essential Experience Required
- Undertaking clinical functional assessments and Occupational Therapy intervention planning implementation and review.
- Working as an Occupational Therapist with people with physical disabilities in the community providing appropriate interventions to enable independence at home.
- Inter-agency and multi-disciplinary work to support collaborative working relationships.
Essential Qualification Required
- Diploma / BSc Occupational Therapy or equivalent Occupational Therapy qualification.
- Registration with Health and Care Professions Council (HCPC).
- SW registration/qualification.
Additional Information
- Working hours: 37 hours per week.
- Location: County Hall North Horsham County Hall Chichester Bridge House Worthing.
- Working arrangements: Hybrid 1 day a week (where feasible).
- Valid full driving licence is essential.
- The role requires a DBS.
