Responsibilities

Job Summary/Purpose:

This position is responsible for providing leadership and clinical pharmacy services including the implementation of System Region and Facility Pharmacy Services initiatives. The incumbent will provide leadership in establishing and maintaining clinical pharmacy programs mentor staff pharmacists to clinical pharmacy activities educate pharmacy residents and students affiliated with the facility provide clinical pharmacy support to one or more Service Lines and collaborate with the facility Pharmacy leadership team in establishing action plans to support department initiatives at the campus level. Ensures that all actions taken and methods to produce the results outlined below incorporate and promote CSHs mission vision and values.

85% or more time is spent:

  • Performs all duties of a registered staff pharmacist working online/staffing
  • Exempt status for the Specialist is the default unless defined by contract or Region standard

What Youll Do:

  • Strategic Planning & Collaboration:Contribute to the development and implementation of both short-term and long-term pharmacy plans aligning with market trends regional initiatives and the overall healthcare system. Collaborate with fellow pharmacists to standardize clinical pharmacy systems for enhanced efficiency and consistency.
  • Mentorship & Development:Help foster a culture of growth and learning by mentoring and coaching pharmacy staff empowering them to expand their clinical skills and engage in scholarly activities across the continuum of care.
  • Education & Advocacy:Educate medical staff on the principles of cost-effective high-quality pharmaceutical care grounded in evidence-based medicine and best practices.
  • Committee Participation:Actively participate in relevant committees sharing your knowledge and experience to contribute to important decisions and initiatives.
  • Regulatory Compliance:Ensure that all clinical pharmacy initiatives adhere to regulatory requirements maintaining the highest standards of safety and compliance.
  • Preceptorship:Guide and mentor Pharmacy Interns and Pharmacy Residents shaping the next generation of pharmacy professionals.
  • Clinical Practice:Provide direct patient care through centralized and/or decentralized staffing applying your clinical expertise to optimize medication therapy.
  • Documentation:Accurately document clinical activities and interventions such as consults patient teachings and other patient-specific information in accordance with established policies and procedures.
  • Medication Management:Interpret medication orders and prescriptions identifying potential drug interactions and ensuring appropriate drug selection. Accurately enter orders into the computer system and dispense medications in a timely manner.
  • Verification & Accuracy:Verify and check medications prepared by other pharmacy personnel ensuring accuracy and safety.
  • Communication & Problem Solving:Communicate drug information effectively to patients and healthcare professionals. Resolve drug-related problems by consulting with other healthcare professionals to achieve optimal patient outcomes. Clarify unclear orders with prescribers and assist in appropriate drug selection.
  • Continuous Learning:Maintain a strong knowledge base to provide clinical services for all age groups served and provide basic clinical services as required by policy and procedure.
  • Technology Proficiency:Utilize automated systems effectively to manage medication-related processes.
  • Technician Support:Provide guidance and direction to pharmacy technicians supporting their activities and performance.
  • Education & Training:Participate in the orientation instruction and training of pharmacy students interns technicians residents pharmacists and other healthcare professionals.
  • Regulatory Compliance:Perform duties in accordance with the requirements of all regulatory and governing bodies.
  • Continuing Education:Complete mandatory education requirements at the system and department levels.
  • Patient Counseling:Provide patient counseling drug information and documentation as appropriate.
  • Other Duties:Complete other duties as assigned within the scope of your licensure.
  • Residency Program Leadership (If Applicable):Serve as the Residency Program Director providing leadership and guidance to the residency program.

Qualifications

Education/Experience:

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree
  • Residency completion or 3 years of clinical experience

License/Certification:

  • Active State Pharmacist License in good standing in the state in which the employee works: PHARM:XX
  • Board Certification (e.g. BCCP BCPS) required within 3 years of hire - any incumbents would not be required to obtain board certification
  • BLS-CPR
  • ACLS
