Job Description

CCLS Case Manager

Supervision: CCLS Program Manager

Education: Bachelors Degree

Summary:

  1. Demonstrate professionalism and a positive attitude in all dealings with members staff and other agencies. Display dependability initiative and enthusiasm; use tact and sensitivity in communicating effectively and respectfully with members other staff and other agencies. The individual must learn understand and practice Independence Center protocols policies language and skills.
  2. A professional demeanor and strong communication skills are essential; this includes verbal written telephone email text social media and other means of communication. This position requires working autonomously and as part of a team. The individual must be intrinsically motivated flexible to shifts in priorities and adaptive to changing work requirements; this includes arriving to work on time following leave request protocols and adhering to an established work schedule.
  3. The employee must demonstrate the ability to function in a computer-oriented office including working knowledge of all relevant software. It is expected to maintain and improve job skills through training education seminars and other activities designed to promote best practices about job duties. Some flexibility in schedule may be required to meet deadlines including extended work.
  • This position will in conjunction with the CCLS program leadership ensure the Comprehensive Community Living Support program functions optimally adhering to regulatory standards and requirements while meeting our members support and rehabilitation needs.
  • Provides or leads crisis intervention and emergency services to members as required.
  • Screen Front Door referrals completes the Consumer Engagement and Offer Notice and determines eligibility for services andparticipation in the program.
  • Aid members in applying for Section 8 & other entitlements.
  • Recognize and act on opportunities to transition members to the most appropriate level of care.
  • Coordinate care by supporting an integrated health paradigm and referring to other services as necessary. Assist consumers in signing up for the Bridge Subsidy Program and the State Referral Network.
  • Provide care coordination that supports the Community Support Team model integrated health model and make referrals toadditional services as needed.
  • Conducts and documents mental health assessments treatment plans safety discharge plans progress notes reports andservice interventions for services rendered.
  • Assess and apply for other supportive housing programs with openings or waiting lists.
  • Conduct case management services including linking members to community resources and providing referrals to meet theirneeds. Assist residents in obtaining public entitlements housing and community resources (RTA Free Ride program foodpantries clothing donations LIHEAP (Low Income Home Energy Assistance Program) PADS etc.)
  • Educate and assist members with applying for entitlements i.e. Supplemental Security Income Social Security DisabilityInsurance Medicare Medicaid and LINK.
  • Provide community support services and assist individuals in achieving and maintaining rehabilitative resiliency and recoverygoals. Services provided include therapeutic interventions that facilitate self-management of illness skill-building identificationand utilization of natural resources and access to community resources.
  • Coordinate transport and attend appointments with mental and physical health medical providers to promote collaborative careand provide input on treatment needs.
  • Conduct non-medical assessments including home safety transportation employment and community/environmentalresources to provide referrals for appropriate services and resolve concerns on the members behalf.
  • Facilitate groups and ensure attendance is maintained in accordance with the agencys expectations.
  • Provide brief supportive and goal-directed counseling to assist members with decisions on housing matters and facilitate processchanges.
  • Facilitate therapeutic and educational services including individual family and group therapies focusing on life skillscommunication coping and conflict management.
  • Per policy provide outreach via phone email in person and text to individuals to remove therapeutic ambivalence to treatmentfollow up on referrals and reduce any remaining barriers.
  • Advocates on behalf of members and makes requisite recommendations as needed. Intervenes and advocates with collateralsources agencies bureaucracies facilities and families on members behalf.
  • Provide crisis intervention when necessary.
  • Other duties as assigned by the supervisor.

Physical Demands:

There are considerable physical demands associated with this position. The employee will have to spend long hours sitting using office equipment and computers which can cause muscle strain. The position may also have to do some light lifting of supplies and materials from time to time.

Requirements

A Bachelors degree in counseling rehabilitation counseling social work education vocational counseling psychology pastoral counseling family therapy or related human service field or a Bachelors degree in any other field with two years of supervised clinical experience in a mental health setting or a high school diploma/GED with a Certificate of Psychiatric Rehabilitation from a DHS-approved program/ CRSS certification through ICB. and two years of experience in a mental health setting.

Experience in community-based services preferred

1-3 years of clinical experience preferred

Experience in Crisis Intervention & De-escalation

Must be self-motivated organized and comfortable in a fast-paced dynamic environment. Experience with clients with serious mental illness substance abuse problems and medically complex needs a plus. Must be able to work with diverse populations with multi-faceted needs. Excellent written and oral communication skills needed with demonstrated ability to engage clients. Efficient computer literacy skills and ability to utilize Microsoft Office. Valid drivers license.
