We are seeking a dedicated and compassionate Wound Care Nurse Practitioner to join our Wound Care team. The ideal candidate will provide high-quality patient care focusing on both preventive and acute health needs. This role requires strong wound care skills the ability to work collaboratively with other healthcare professionals and a commitment to improving patient outcomes.
Duties
- Conduct comprehensive assessments of patients including physical examinations and medical histories.
- Develop and implement individualized care plans based on patient needs.
- Manage acute pain and provide appropriate interventions for pain relief.
- Provide catheter care and monitor patients for complications.
- Maintain accurate medical records and documentation in compliance with healthcare regulations.
- Collaborate with interdisciplinary teams to ensure coordinated care for patients.
- Educate patients and families about health conditions treatment options and preventive measures.

- Proficient in wound care practices ensuring timely interventions.
- Familiarity with durable medical equipment (DME) management and recommendations.
