Job Summary:
This role requires an experienced Community Support Worker to deliver high-quality person-centred care and support to adults with learning disabilities.
The post involves lone working within the community supporting multiple service users to promote independence inclusion and improved quality of life.
The successful candidate will be adaptable empathetic and confident working in a variety of home and community-based settings.
Key Duties/Accountabilities (Sample):
Provide direct practical and emotional support to individuals with learning disabilities in the community.
Promote independence and choice by enabling service users to develop daily living skills and take part in activities.
Work autonomously in lone-working situations while ensuring consistent high-quality care.
Maintain accurate and up-to-date records and documentation in line with safeguarding confidentiality and GDPR requirements.
Communicate effectively with service users families carers and other professionals.
Assist in the implementation and review of care/support plans.
Respond appropriately to emergencies and safeguarding concerns following policy and procedure.
Support service users with personal care tasks where required.
Actively promote equality diversity and inclusion in service delivery.
Skills/Experience:
Previous experience working with adults with learning disabilities is essential.
Proven ability to lone work confidently and responsibly in a community setting.
Excellent interpersonal and communication skills.
Strong problem-solving and decision-making skills.
Ability to manage time effectively and prioritise workload.
Understanding of safeguarding procedures and confidentiality requirements.
Commitment to delivering person-centred care and promoting service user rights.
Ability to use basic IT systems for record-keeping and communication.
NVQ Level 2/3 in Health and Social Care (or equivalent) is desirable.
Additional Information:
This is a temporary full-time role (32.5 hours per week).
Working hours are 09:00 to 15:30 Monday to Friday.
An enhanced DBS and references will be required prior to appointment.
