The Program Manager Palliative Care will lead a collaborative team to deliver innovative palliative care services in Mississauga ensuring high-quality patient-centered care. Key responsibilities include:
- Develop and implement innovative palliative care programs optimizing service delivery and care transitions across the health system
- Lead mentor and manage a multidisciplinary team of nurses PSWs and allied health professionals fostering a culture of excellence and continuous learning
- Ensure program quality and efficiency through data-driven insights performance monitoring and implementation of best practices
- Collaborate with internal and external stakeholders to enhance palliative care services and support seamless care integration
- Oversee recruitment onboarding and ongoing professional development of team members
- Address quality safety and risk issues developing and implementing improvement plans as needed
- Manage client satisfaction addressing concerns promptly and effectively
Qualifications :
- Current CNO certificate as Registered Nurse degree preferred
- Minimum of 5 years of recent experience with progressive responsibilities as a Nursing Manager preferably in a community setting
- Successful completion of one or a combination of the following required:
- Canadian Nursing Association (CNA) certification in Palliative Care Nursing or enrollment in CNA certification program in Palliative Care Nursing; or
- Learning Essentials Approaches to Palliative and End of Life Care (LEAP); or
- Fundamentals of Palliative Care
- The Comprehensive Advanced Palliative Care Education (CAPCE)
- Experience managing in a union environment is an asset
- Solid knowledge of the principles practices and methods related to leading multidisciplinary teams
- Experience with program development implementation and evaluation
- Valid G drivers license and reliable vehicle
