Be part of something truly special. As a key team member in our PACE program youll help redefine aging by supporting older adults in living with dignity purpose and independence. Youll build trusting relationships assess individual needs and coordinate care that honors the whole person. Working alongside an interdisciplinary team youll craft personalized care plans that empower participants to thrive while safely and confidently aging in place.
The Heart of PACE:
At LifeCircles PACE we believe older adults deserve to age with dignity support and connection. As a nonprofit community-based program we offer a coordinated system of care that brings together medical social and personal services - each one thoughtfully managed by our dedicated interdisciplinary team. With physicians nurses social workers therapists and more working side by side we deliver comprehensive person-centered care that helps Participants remain safe healthy and independent while aging in place.
Primary Responsibilities:
- The Spiritual Coordinator provides inclusive compassionate spiritual and emotional support to individuals of all faiths belief systems and cultural backgrounds. The Coordinator offers one-on-one presence facilitates meaningful programming and fosters a welcoming environment that honors the unique spiritual needs of each participant. The Coordinator plays a key role in supporting grief life transitions and end-of-life care through reflective practices. In collaboration with participants families and interdisciplinary team members the Coordinator ensures holistic person-centered care that integrates spiritual and cultural preferences.
How youll stand out:
- Bachelors or Masters in Theology Religious Studies Spiritual Care Pastoral Counseling Social Work or related field
- Clinical Pastoral Education (CPE) training in grief counseling trauma-informed care or end-of-life training
- Must possess a valid drivers license and maintain proper auto insurance
- Experience in inter-faith or multi-faith environments demonstrating respect for diverse belief systems and the ability to navigate religious pluralism
