Bilingual Nurse Practitioner (NP)

This Nurse Practitioner role is part of a mission-driven healthcare organization that delivers equitable value-based care to the Medicare population fostering wellness and prevention through team-based support and local engagement.
Salary and Benefits
- Base Salary: $105000 $135000 (commensurate with experience and region)
- Incentive Bonus: $15000 $25000 annually based on quality outcomes
- Physician Partnership Track available
- 6 Weeks of Paid Time Off (inclusive of vacation holidays and CME)

Responsibilities
- Full-Time Schedule: Monday to Friday 8:00 AM 5:00 PM
- Patient Load: 1316 patients per day
- Appointment Duration:
- Routine Visit: 20 minutes
- New/Complex Visit: 40 minutes
- Preventive Value-Based Care Model
- Support Team Includes:
- Medical Assistant
- Scribe
- Social Worker
- Onsite Laboratory and Phlebotomy
- Specialist Access via RubiconMD
- EMRs: Canopy and Greenway
- Participation in Community Health Events and Wellness Programs
- Patient Transportation Within a 5-Mile Radius
- Collaborative Setting with 35 Providers Per Clinic
Requirements
- Master of Science in Nursing Doctor of Nursing Practice or equivalent accredited NP program
- Active Illinois NP License and DEA License
- National Board Certification in:
- Family Nurse Practitioner (FNP)
- Adult-Gerontology Primary Care NP (AGPCNP)
- Adult NP (ANP)
- Gerontological NP (GNP)
- Bilingual Spanish Required
- Experience with HCC Documentation ICD-10 Coding and Health Risk Assessments (Preferred)
