Job Description
Licensed Clinical Social Worker (LCSW) Care Management
Department: Care Management
Location: Athena Medtech
Schedule: Monday Friday 08:00 16:30 40 hours/week
Licensure Requirement: Active Maryland LCSW license required prior to interview
Experience Required: Minimum 3 years including recent acute hospital discharge planning
Athena Medtech is seeking a highly experienced and compassionate Licensed Clinical Social Worker (LCSW) to join our Care Management team. This vital role focuses on supporting patients through comprehensive discharge planning and psychosocial services ensuring a smooth and safe transition from hospital to home or other care settings.
Key Responsibilities-
Lead discharge planning processes including development and implementation of individualized care plans.

Conduct resident assessments and coordinate interdisciplinary care efforts.

Complete administrative documentation such as medical forms evaluations reports transportation coordination and charting.

Participate in interdisciplinary team meetings to promote integrated patient-centered care.

Communicate effectively with patients and families regarding discharge status planning updates and care goals throughout hospitalization.

Educate patients and families on Medicare Medicaid and other financial assistance programs.

Serve as a consultant to clinical staff for psychosocial challenges discharge barriers and care transitions following Skilled Nursing Unit (SNU) or Acute Rehab Unit (ARU) stays.

Masters degree in Social Work from an accredited program.

Current LCSW license in the state of Maryland (must be active before interview).

Minimum of 3 years of professional social work experience including recent experience in acute care hospital discharge planning.

Strong knowledge of community resources healthcare systems and transitional care services.

Excellent interpersonal organizational and communication skills.
