LPN - Continuing Care
About Facility:
o Skilled Nursing Facility
o Radius rule: Travelers permanent address must be 50 miles outside of home unit
Department Information:
o Beds: 70
o Minimum Years of Experience required 18 months in SNF/LTC Rehab facility Will you accept a first-time traveler Not preferred
Patient Types/Common Diagnoses:
o Short-term and Long-term services offered to patients recovering from an illness injury or accident
o Stroke Amputation Neurological Disorders s/p Joint Replacement etc
o No memory care
Patient Ratios:
o LPN: 1:18-20 Days; 1:26 Nights
Required License National Certification Certs (BLS ACLS etc.) or other position specific requirements
o LPN: BLS
Skills required:
o Previous Long-Term Care exp to include: Trach management (refresher provided during orientation) foley care wound experience dressing changes IV starts/management tube feedings
Support within the Department:
o Nursing Leadership
o Admin Nurse will do any admission admissions will be approx. 1-2 per day
o HCT
o Therapies
o Case Management
Technology/Equipment:
o EMR: EPIC
o IV Pump: Alaris
o Med dispensing: Omnicell
Floating:
o Within Continuing Care and scope of practice
o Will be required to float per region needs will require floating to all facilities listed below as needed within scope of practice DECLINE if not agreeable
o Rehab Region - Cleveland Pines Huntersville Oaks Jesse Helms Sardis Oaks and Stanly Manor
o If floating mid-shift Traveler will be paid for the time traveling between facilities (will remain on the clock)
Orientation (facility and unit):
o 2 days departmental
Scheduling:
o 12-hour shifts (36 hours a week)
o Weekend rotation: every other
o No call
o Holiday Expectations: Travelers will work 75% of holidays
o RTO: Can approve up to 6 days off excluding holidays must add time to the end of the contract if unable to meet contracted hours
o Can NOT pick up extra shift but can have 48hr/week contract
