At Central City Concern (CCC) we are dedicated to ending homelessness and fostering long-term stability for Portlands most vulnerable populations. Through our Veterans Grant & Per Diem (GPD) Program we provide transitional housing and comprehensive support to formerly homeless veterans guiding them toward self-sufficiency and a brighter future.

Schedule: Monday - Friday 8:00am - 4:30pm
Compensation: starting at $22.19/hr
MINIMUM QUALIFICATIONS:
- High School Diploma or GED required and six months of experience working with the homeless population behavioral health environment human services or related environment; OR 6 months experience in a high-volume customer service role.
- Must possess a current drivers license pass a DMV background check and be designated an acceptable driver as outlined in Central City Concerns Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training.
- Must not actively be on parole or probation.
- Knowledge of area housing resources human service agencies and community groups.
- Proficient in Microsoft Office Suite (Outlook Word Excel PowerPoint etc.)
- Must embrace the mission and philosophy of recovery and client centered treatment.
- Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern.
- Sufficient manual dexterity and physical ability to perform duties.
- Ability to adhere to Central City Concerns drug-free workplace which encourages a safe healthy and productive work environment and strictly complies with the Drug-Free Work Place Act of 1988. An employee shall not in the workplace unlawfully manufacture distribute dispense possess or use a controlled substance or alcohol.

PREFERRED QUALIFICATIONS:
- Experience working with homeless individuals and/or Veterans or Military.
- Military Veteran with General under Honorable or Honorable Discharge.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide assistance to customers in working toward the program goals of income enhancement seeking permanent housing and developing self-sufficiency.
- Provide case management support for higher acuity clients with emergent needs.
- Provide client assessment guidance in developing goal plans and monitoring client progress.
- Provide ongoing individual support and encouragement to clients and conduct quarterly reviews of their progress.
- Educate customers about employment assistance options service/non-service connected and social security benefits and community engagement opportunities.
- Collaborate with community resource agencies to remove barriers provide intervention and make referrals for issues facing customers in a variety of areas including but not limited to employment substance abuse health finances and legal/criminal issues.
- Identify community and individual issues which could result in a change in a customers status and work to achieve appropriate resolutions.
- Connect with community landlords to create viable rental opportunities for customers as they transition out of the program.
- Work with internal and VA service providers to assure coordinated and streamlined services.
- Input and maintain accurate records of customer progress in working toward individualized program goals using MS Word Access and other software.
- Participate in compliance quality assurance and quality improvement activities as directed.
- Facilitate vocational classes workshops and events as needed.
- Conduct weekly group meetings to encourage community building and mutual support among customers.
- Attend appropriate community professional meetings trainings and events.
- Attend all mandatory CCC trainings in a timely manner.
- Adhere to all state and federal privacy and security regulations applicable to the program and to CCC policies and agreements regarding confidentiality privacy and security. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department as well as to your supervisor or their designee.
