Position Overview:
Under the direct supervision of the Outreach Program Supervisor the Community Health Worker is responsible for supporting Harris County Public Healths outreach activities and serves as the key liaison to educate promote and connect community members to all HCPH programs/services specifically on behalf of the Lead Poisoning Prevention and Substance Use Prevention Programs. This position will primarily work in the community through various outreach activities within Harris County while also completing in-office tasks/projects to support overall outreach team objectives.
The Community Health Worker is expected to establish and maintain effective working relationships and foster teamwork in a diverse environment.
Duties and Responsibilities:
Responsible for diverse outreach education and community coordination activities such as canvassing local businesses/organizations attending community events conducting outreach phone calls and giving presentations.

Education:
High School diploma.
Knowledge Skills and Abilities:
Intermediate proficiency in Microsoft Office.
Must communicate clearly and effectively both orally and in writing.

This is a full-time grant- funded position.
This job operates in both field and office environment. This role routinely uses standard office equipment such as computers phones photocopiers filing cabinets and fax machines.
Employment may be contingent on passing a drug screen and meeting other standards.
